Objective: To identify the seroprevalence of infectious serological markers in blood donors at the Blood Bank of the National Institute of Neoplastic Diseases (INEN) during the 2020-2024 period. Material and methods: A basic, quantitative research study was conducted with a non experimental, observational, descriptive, retrospective, and cross-sectional design. The population and sample were census-based, consisting of 131,911 records of screened donors, with no exclusion by subsampling. Data were obtained through documentary analysis of the institutional database, evaluating the reactivity of markers for HIV, HBsAg, Anti-HBc, HCV, Syphilis, Chagas, and HTLV-I/II, processed using the chemiluminescent immunoassay (CMIA) technique. Descriptive statistics were used for analysis, calculating frequencies and percentages. Results: It was evidenced that the Anti-HBc marker showed the highest global seroprevalence at 2.77%, followed by syphilis at 1.36%. Other markers showed lower reactivity, such as HCV (0.80% in 2024) and HIV, which decreased to 0.16% at the end of the period. Likewise, the prevalence of co-infection was low (0.42%), with the most frequent combination being Anti-HBc + HBsAg (34.53%), followed by syphilis + Anti-HBc (22.48%). Conclusion: It was determined that seroprevalence at INEN is characterized by the predominance of hepatitis B (Anti-HBc) and syphilis markers, confirming the endemicity of the former and the need to strengthen donor selection filters and hemovigilance strategies for sexually transmitted infections.en_US
