El objetivo fue determinar la asociación entre la morfología de la papila duodenal mayor y la canulación biliar difícil en pacientes sometidos a CPRE en un hospital nivel III de Lima durante el periodo 2024–2025. Se desarrolló un estudio de enfoque cuantitativo, método hipotéticodeductivo, tipo básico, observacional, analítico y retrospectivo, con diseño no experimental, transversal y correlacional-analítico. La población estuvo conformada por pacientes adultos sometidos a colangiopancreatografía retrógrada endoscópica en el Hospital Nacional Dos de Mayo; de 200 registros revisados, 197 integraron la muestra analítica principal y 194 el modelo ajustado, seleccionados mediante muestreo probabilístico aleatorio simple. La técnica fue la revisión documental retrospectiva y el instrumento una ficha estructurada elaborada por el investigador, aplicada a informes endoscópicos, registros fluoroscópicos e historias clínicas. La canulación biliar difícil se definió mediante los criterios ESGE 5-5-2. Los resultados mostraron predominio de morfología tipo I (59,4%), seguida de tipo III (21,8%), tipo IV (12,7%) y tipo II (6,1%); la proporción de canulación difícil fue 34,0%. En el análisis ajustado, las morfologías tipo II (RPa=1,76; IC95%:1,03–3,01; p=0,037), tipo III (RPa=2,25; IC95%:1,47–3,45; p<0,001) y tipo IV (RPa=2,04; IC95%:1,26–3,30; p=0,004) se asociaron significativamente con canulación difícil frente al tipo I. Además, el sexo masculino, la mayor edad y el menor diámetro del colédoco conservaron significancia estadística. Se concluyó que la morfología papilar del tipo II, III y IV se asociaron a mayor dificultad de canulación biliar respecto a la morfología tipo I y resulta útil para anticipar complejidad técnica durante la CPRE.
Resumen
The objective was to determine the association between the morphology of the major duodenal papilla and difficult biliary cannulation in patients undergoing ERCP at a tertiary hospital in Lima during the 2024–2025 period. A quantitative study was conducted using the hypothetico-deductive method, with a basic, observational, analytical, and retrospective approach, and a non experimental, cross-sectional, correlational-analytical design. The population consisted of adult patients who underwent endoscopic retrograde cholangiopancreatography at Hospital Nacional Dos de Mayo; of 200 reviewed records, 197 formed the main analytical sample and 194 were included in the adjusted model, selected through simple random probability sampling. The technique was retrospective documentary review, and the instrument was a structured data collection form designed by the researcher and applied to endoscopic reports, fluoroscopic records, and medical charts. Difficult biliary cannulation was defined using the composite 5-5-2 criterion. The results showed a predominance of type I papilla (59.4%), followed by type III (21.8%), type IV (12.7%), and type II (6.1%); the proportion of difficult cannulation was 34.0%. In the adjusted analysis, type II (aPR=1.76; 95%CI:1.03–3.01; p=0.037), type III (aPR=2.25; 95%CI:1.47–3.45; p<0.001), and type IV papilla (aPR=2.04; 95%CI:1.26–3.30; p=0.004) were significantly associated with difficult cannulation compared with type I. In addition, male sex, older age, and smaller common bile duct diameter remained statistically significant. It was concluded that non regular papillary morphology, together with selected clinical and anatomical characteristics, is associated with greater difficulty in biliary cannulation and is useful for anticipating technical complexity during ERCP.
