Utilidad del monitoreo electrónico fetal en la identificación y manejo de restricción del crecimiento intrauterino: reporte de un caso clínico

dc.description.abstractLa restricción del crecimiento intrauterino (RCIU) es una condición asociada a un mayor riesgo de complicaciones perinatales, incluidas hipoxia fetal, parto prematuro y muerte neonatal. Su detección oportuna durante el control prenatal y el monitoreo fetal intraparto son fundamentales para reducir la morbilidad. El monitoreo electrónico fetal, mediante el test no estresante (NST) y el test estresante (TST), permite evaluar el bienestar fetal y guiar decisiones clínicas. Presentación del caso: Gestante de 30 años, G2P1001, con embarazo de 40 semanas por ecografía del primer trimestre. Acude al servicio de emergencia por contracciones uterinas de 5 horas. Refirió movimientos fetales conservados y sin pérdida de líquido amniótico. Durante el control prenatal se evidenció peso fetal estimado en percentil 5. Se realizaron NST (reactivo) y TST (negativo con desaceleraciones tempranas). En el trabajo de parto se observaron desaceleraciones tardías. A las 11:34 se atendió parto vaginal espontáneo de una recién nacida viva, Apgar 9/9, peso 2650 g, talla 47 cm, e índice ponderal de 2.55, compatible con restricción del crecimiento fetal tardía. Discusión: El caso evidencia el valor del monitoreo fetal en gestaciones con RCIU. Las pruebas no invasivas permitieron un seguimiento adecuado, y la identificación de desaceleraciones tardías en el expulsivo orientó a una vigilancia estricta hasta la resolución del parto. Conclusión: El monitoreo electrónico fetal es clave en la atención intraparto de embarazos con RCIU, permitiendo intervenciones oportunas que mejoran los desenlaces perinatales.es_ES
dc.description.abstractIntrauterine growth restriction (IUGR) is a condition associated with an increased risk of perinatal complications, including fetal hypoxia, preterm birth, and neonatal death. Early detection during prenatal care and intrapartum fetal monitoring are essential to reduce morbidity. Electronic fetal monitoring, through the non-stress test (NST) and contraction stress test (CST), allows assessment of fetal well-being and guides clinical decision-making. Case Presentation: A 30-year-old pregnant woman, G2P1001, at 40 weeks of gestation according to first-trimester ultrasound, presented to the emergency department with uterine contractions lasting 5 hours. She reported preserved fetal movements and no loss of amniotic fluid. During prenatal follow-up, the estimated fetal weight was found to be at the 5th percentile. An NST (reactive) and a CST (negative with early decelerations) were performed. During labor, late decelerations were observed. At 11:34 a.m., a spontaneous vaginal delivery of a live female newborn occurred, with Apgar scores of 9/9, birth weight of 2650 g, length of 47 cm, and a ponderal index of 2.55, findings compatible with late fetal growth restriction. Discussion: This case highlights the value of fetal monitoring in pregnancies complicated by IUGR. Non-invasive tests allowed adequate follow-up, and the identification of late decelerations during the expulsive phase guided strict surveillance until delivery. Conclusion: Electronic fetal monitoring is essential in the intrapartum management of pregnancies complicated by IUGR, allowing timely interventions that improve perinatal outcomes.en_US
