Utilidad de la RPCa en el tamizaje del factor V Leiden en pacientes con TVP, hospital case, Arequipa 2025

Abstract

La trombosis venosa profunda (TVP) es la formación de coágulos sanguíneos principalmente en miembros inferiores y su causa se hace más frecuente en mutaciones puntuales dirigidas a factores de coagulación sanguínea entre ellas el factor V que al mutar se denomina Factor V Leiden (FVL). En nuestro país la TVP tiene mucha importancia con una incidencia de 117 de cada 100000 personas al año y viene hacer la tercera causa entre las enfermedades cardiovasculares y con una alta morbilidad, que de no ser diagnosticada y tratada a tiempo podría desencadenar tromboembolismos que alcanzarían al pulmón complicando la salud del paciente. Su diagnóstico temprano ayudaría a reducir el riesgo de mortalidad. Por tal motivo el demostrar la presencia de la mutación del FVL a través de la prueba de resistencia a la proteína C activada (RPCa) que es un test de bajo costo y con una alta sensibilidad y especificidad beneficiaria a nuestros pacientes referidos al servicio de Hematología y al clínico en mejorar la calidad de vida del paciente.

Deep vein thrombosis (DVT) is the formation of blood clots, primarily in the lower limbs. Its cause is increasingly common in point mutations affecting blood clotting factors, including factor V, which, when mutated, is called Factor V Leiden (FVL). In our country, DVT is very significant, with an incidence of 117 per 100,000 people per year. It is the third leading cause of cardiovascular disease and has a high morbidity rate. If not diagnosed and treated promptly, it can trigger thromboembolisms that can reach the lungs, complicating the patient's health. Early diagnosis helps reduce the risk of mortality. Therefore, demonstrating the presence of the FVL mutation through the activated protein C (aPC) resistance test, which is a low-cost test with high sensitivity and specificity, benefits our patients referred to the Hematology service and the clinician by improving the patient's quality of life.
