Factores de acceso a servicios de salud que se relacionan con la falta de búsqueda de atención médica en un proveedor de salud por síntomas de infección respiratoria aguda en menores de 5 años en el Perú según la encuesta demográfica y de salud familiar (ENDES) 2023

Abstract

El objetivo de la investigación fue identificar la relación entre los factores de acceso a servicios de salud y la falta de búsqueda de atención médica por síntomas de infección respiratoria aguda (IRA) en niños menores de 5 años en el Perú, según la ENDES 2023.Se realizó un estudio cuantitativo, hipotético-deductivo, no experimental, transversal y correlacional, basado en el análisis secundario. Se aplicaron análisis descriptivos, bivariados y multivariados mediante regresión logística binaria, con α = 0,05, estimando OR e IC 95%. Entre 13 894 niños con síntomas de infección respiratoria aguda (IRA), el 69,4% presentó falta de búsqueda de atención médica. En el análisis multivariado se identificó asociación significativa para la etnicidad achacani (OR= 0,344; IC 95%: 0,125-0,95; p=0,04), para el nivel educativo superior del cuidador (OR = 0.703; IC 95 %: 0.582–0.849; p < 0.001) y para la presencia de medicamentos en el hogar (OR= 27,439; IC95%: 24,205-31,105; p<0,001). En conclusión, los indicadores asociados a la falta de búsqueda de atención médica fueron la etnicidad del cuidador y el nivel educativo, mientras que la presencia de algún medicamento en el hogar actuó como un factor protector frente a este evento; Los demás indicadores evaluados no demostraron asociación significativa con la falta de búsqueda de atención médica.

This study aimed to identify factors associated with healthcare-seeking behavior for symptoms of acute respiratory infection (ARI) in children under five years of age in Peru, based on data from the 2023 Demographic and Family Health Survey (ENDES). A quantitative, hypothetico-deductive, non-experimental, cross-sectional, and correlational design was used, based on secondary analysis of the survey database. Descriptive, bivariate, and multivariate analyses were conducted using binary logistic regression with a significance level of 5%, estimating odds ratios (OR) with 95% confidence intervals. Among 13,894 children with respiratory symptoms, 69.4% did not seek medical care. In the multivariate analysis, significant associations were found for Achacani ethnicity (OR = 0.344; 95% CI: 0.125–0.95; p = 0.04), higher caregiver educational level (OR = 0.703; 95% CI: 0.582–0.849; p < 0.001), and the presence of medications in the household (OR = 27.439; 95% CI: 24.205–31.105; p < 0.001). In conclusion, caregiver ethnicity and educational level were associated with a lack of healthcare-seeking for ARI symptoms, whereas having medications available at home acted as a protective factor against this outcome; The remaining indicators did not show statistically significant associations.
