Tipificación ABO directa e inversa en la determinación de discrepancias en pacientes transfundidos del Hospital de Apoyo Chepén la Libertad de Enero – diciembre 2024

Abstract

La tipificación sanguínea ABO es un procedimiento clave en medicina transfusional, ya que asegura la compatibilidad entre donantes y receptor, previniendo reacciones hemolíticas graves. Este proceso incluye dos pruebas: la directa, que detecta antígenos en los eritrocitos, y la inversa, que identifica anticuerpos en el suero. Sin embargo, pueden presentarse discrepancias entre ambas, lo que representa un riesgo clínico si no se detectan a tiempo. Estudios internacionales reportan una prevalencia de discrepancias entre 0.27% y 0.38%, asociadas a factores como transfusiones recientes, enfermedades hematológicas, inmunodeficiencias y errores técnicos. En el Perú, aún existen falencias en protocolos, capacitación del personal y sistematización de estos casos. En el Hospital de Apoyo Chepén no se han realizado investigaciones previas al respecto, a pesar de efectuarse transfusiones con frecuencia. Por ello, el presente estudio busca determinar las discrepancias ABO directa e inversa en pacientes atendidos entre enero y diciembre de 2024, con el objetivo de fortalecer la seguridad transfusional.

ABO blood typing is a key procedure in transfusion medicine, as it ensures compatibility between donors and recipients, preventing severe hemolytic reactions. This process includes two tests: the direct test, which detects antigens on red blood cells, and the reverse test, which identifies antibodies in serum. However, discrepancies between the two can occur, representing a clinical risk if not detected in time. International studies report a prevalence of discrepancies between 0.27% and 0.38%, associated with factors such as recent transfusions, hematological diseases, immunodeficiencies, and technical errors. In Peru, there are still shortcomings in protocols, staff training, and the systematization of these cases. At the Chepén Support Hospital, no prior research has been conducted on this matter, despite frequent transfusions. Therefore, the present study seeks to determine the direct and inverse ABO discrepancies in patients treated between January and December 2024, with the aim of strengthening transfusion safety.
