Marcadores inmunohistoquímicos y su relación con el tratamiento de Cáncer de mama en pacientes del Hospital Santa Rosa, Lima, 2024

Abstract

Introducción: El cáncer de mama es una enfermedad heterogénea, donde el manejo terapéutico se basa en sus características biológicas. Los marcadores inmunohistoquímicos, tales como los receptores hormonales (estrógeno y progesterona), HER-2 y Ki-67, son claves para clasificar la enfermedad en diversos subtipos moleculares, cada uno con distintos comportamientos clínicos y pronósticos. Evaluar estos marcadores es crucial para seleccionar el tratamiento más adecuado, que puede incluir hormonoterapia, terapia dirigida o quimioterapia, favoreciendo así una atención más personalizada. El objetivo del estudio fue determinar la relación entre los marcadores inmunohistoquímicos y el tratamiento del cáncer de mama. Se trató de un estudio aplicado, no experimental y de enfoque cuantitativo, con una población de 85 pacientes diagnosticadas con cáncer de mama en el Servicio de Anatomía Patológica del Hospital Santa Rosa durante 2024. La recolección de datos se realizó mediante una ficha de datos y el análisis se llevó a cabo utilizando Excel y el software estadístico SPSS. Los resultados mostraron que el subtipo luminal B fue el más prevalente con 42 casos que representan el 49.9% del total, seguido del subtipo luminal A con un 21.2%. Los subtipos Her-2 y triple negativo fueron menos frecuentes, con 17.6% y 11.8%, respectivamente. En cuanto al tratamiento se evidencia que la hormonoterapia con quimioterapia fue más frecuente en 42 casos en las mujeres con cáncer de mama con un porcentaje del 49,4%, seguido de la hormonoterapia con 21,2%, terapia her 17,6% y quimioterapia 11,8%. Las conclusiones del estudio destacan que el perfil inmunohistoquímico predominante es el subtipo luminal B. Estos hallazgos subrayan la importancia de la caracterización inmunohistoquímica como una herramienta esencial para entender el comportamiento del cáncer de mama.

Introduction: Breast cancer is a heterogeneous disease, where therapeutic management is based on its biological characteristics. Immunohistochemical markers, such as hormone receptors (estrogen and progesterone), HER-2, and Ki-67, are key to classifying the disease into various molecular subtypes, each with different clinical behaviors and prognoses. Evaluating these markers is crucial for selecting the most appropriate treatment, which may include hormone therapy, targeted therapy, or chemotherapy, thus favoring more personalized care. The objective of the study was to determine the relationship between immunohistochemical markers and breast cancer treatment. It was an applied, non experimental study with a quantitative approach, involving a population of 85 patients diagnosed with breast cancer at the Pathology Department of Santa Rosa Hospital during 2024. Data collection was carried out using a data sheet, and the analysis was conducted using Excel and the statistical software SPSS. The results showed that the luminal B subtype was the most prevalent with 42 cases, representing 49.9% of the total, followed by the luminal A subtype with 21.2%. The Her2 and triple-negative subtypes were less frequent, with 17.6% and 11.8%, respectively. Regarding treatment, it is evident that hormone therapy with chemotherapy was more frequent in 42 cases among women with breast cancer, with a percentage of 49.4%, followed by hormone therapy with 21.2%, HER therapy with 17.6%, and chemotherapy with 11.8%. The conclusions of the study highlight that the predominant immunohistochemical profile is the luminal B subtype. These findings underscore the importance of immunohistochemical characterization as an essential tool for understanding breast cancer behavior.
