Perfil clínico-laboratoriales y su asociación con la preeclampsia en gestantes del instituto nacional materno perinatal durante el periodo 2021 a 2023

Abstract

Introducción: La preeclampsia es una complicación hipertensiva del embarazo que representa un riesgo para la salud materno-perinatal. Factores obstétricos y sociodemográficos influyen en su desarrollo, por lo que un seguimiento adecuado es crucial. Objetivo: Determinar la asociación entre el perfil clínico-laboratorial y la preeclampsia en gestantes del Instituto Nacional Materno Perinatal durante 2021-2023. Materiales y Métodos: Estudio retrospectivo caso-control en gestantes del INMP, analizando 309 historias clínicas mediante estadísticas univariadas, bivariadas y regresión logística (p < 0.05) en el programa STATA v14. Resultados: Las gestantes con preeclampsia presentaron más antecedentes de preeclampsia (28.2% vs. 10.2%, p < 0.001) y enfermedad renal (16.5% vs. 4.9%, p = 0.001). Estos factores aumentaron el riesgo de preeclampsia 3.6 y 3.83 veces, respectivamente. Los controles prenatales adecuados redujeron el riesgo en un 49% (p = 0.022). En cuanto a los laboratorios, la trombocitopenia (86.4% vs. 35.9%, p < 0.001), disfunción renal (5.8% vs. 1%, p = 0.018) y alteraciones hepáticas (p < 0.001) fueron factores de riesgo significativos. Conclusiones: Antecedentes de preeclampsia, enfermedad renal y trombocitopenia aumentan el riesgo de recurrencia, mientras que un adecuado seguimiento prenatal reduce este riesgo.

Introduction: Preeclampsia is a hypertensive complication of pregnancy that poses a risk to maternal and perinatal health. Obstetric and sociodemographic factors influence its development, making adequate monitoring crucial. Objective: To determine the association between clinical-laboratory profile and preeclampsia in pregnant women at the National Maternal Perinatal Institute during 2021-2023. Materials and Methods: A retrospective case-control study was conducted on pregnant women at the INMP, analyzing 309 clinical records using univariate, bivariate statistics, and logistic regression (p < 0.05) in STATA v14. Results: Pregnant women with preeclampsia had more previous preeclampsia (28.2% vs. 10.2%, p < 0.001) and renal disease (16.5% vs. 4.9%, p = 0.001). These factors increased the risk of preeclampsia by 3.6 and 3.83 times, respectively. Adequate prenatal care reduced the risk by 49% (p = 0.022). Regarding laboratory results, thrombocytopenia (86.4% vs. 35.9%, p < 0.001), renal dysfunction (5.8% vs. 1%, p = 0.018), and hepatic alterations (p < 0.001) were significant risk factors. Conclusions: A history of preeclampsia, renal disease, and thrombocytopenia increases the risk of recurrence, while proper prenatal care reduces this risk.
