The use of ketoanalogs in patients with chronic kidney disease reduces serum urea without compromising nutritional status. The present secondary research, entitled “Critical Review”: Effect of the use of ketoanalogs on serum urea in adults with advanced chronic kidney disease, aimed to identify studies related to the clinical question: What is the effect of the use of ketoanalogs on blood urea in adults with advanced chronic kidney disease? The Evidence-Based Nutrition (NuBE) methodology was used. The search for information was carried out in PUBMED, MEDLINE, SCIELO, SCOPUS, and SCIENCE DIRECT, finding 18 articles, 10 of which were selected and evaluated by the CASPE critical reading tool. Finally, the systematic review and meta-analysis entitled "Efficacy and safety of ketoanalogue supplementation combined with protein-restricted diets in advanced chronic kidney disease: a systematic review and meta-analysis, which has a level of evidence A II and a strong recommendation grade, according to the researcher's experience. The critical commentary concluded that there is sufficient evidence to determine the effect of the use of ketoanalogues, with this intervention significantly increasing the research variable score with an SMD of 0.99 (95% CI -1.43, -0.56; P < 0.001). An average very low protein diet of 0.03 g–0.04 g/kg/d and total protein with ketoanalogs of 0.55 g–0.6 g/kg/d was used for 13 months.
