Desempeño de pruebas de mezclas para identificar inhibidores al Factor VIII en pacientes con TTPA prolongado Hospital Virgen de la Puerta Trujillo, 2024

Abstract

El presente proyecto de investigación tiene como objetivo evaluar el desempeño de las pruebas de mezcla en la identificación de inhibidores del factor VIII en pacientes con tiempo de tromboplastina parcial activado (TTPa) prolongado. Esta problemática es relevante en el ámbito clínico, ya que un TTPa prolongado puede deberse tanto a deficiencias de factores de coagulación como a la presencia de inhibidores, lo que dificulta el diagnóstico adecuado y oportuno. El estudio se desarrollará en un hospital de referencia, considerando una población de pacientes con alteraciones en el TTPa. Se empleará un enfoque cuantitativo, de tipo descriptivo y analítico, utilizando la prueba de mezcla inmediata e incubada como herramienta principal para diferenciar entre deficiencias de factores o presencia de inhibidores. Asimismo, los resultados se analizarán mediante indicadores como el índice de Rosner. Se espera determinar la frecuencia de pacientes con inhibidores del factor VIII y evaluar la eficacia diagnóstica de la prueba de mezcla. Los resultados permitirán mejorar la interpretación de pruebas de coagulación, optimizar el diagnóstico diferencial y contribuir a una mejor toma de decisiones clínicas en el manejo de trastornos hemorrágicos.

This research project aims to evaluate the performance of mixing tests in identifying factor VIII inhibitors in patients with prolonged activated partial thromboplastin time (aPTT). This issue is relevant in the clinical setting, since a prolonged aPTT can be due to both coagulation factor deficiencies and the presence of inhibitors, which hinders accurate and timely diagnosis. The study will be conducted in a referral hospital, considering a population of patients with aPTT abnormalities. A quantitative, descriptive, and analytical approach will be used, employing the immediate and incubated mixing test as the main tool to differentiate between factor deficiencies and the presence of inhibitors. The results will be analyzed using indicators such as the Rosner index. The study aims to determine the frequency of patients with factor VIII inhibitors and evaluate the diagnostic efficacy of the mixing test. The results will allow for improved interpretation of coagulation tests, optimize differential diagnosis, and contribute to better clinical decision-making in the management of bleeding disorders.
