Índice neutrófilo-linfocito e índices plaquetarios como predictores de gravedad del dengue en adultos de Iquitos, 2026

Abstract

El dengue constituye un importante problema de salud pública en la región amazónica peruana, donde la identificación temprana de pacientes con riesgo de evolución grave representa un desafío para el primer nivel de atención. El presente proyecto tiene como objetivo evaluar si el índice neutrófilo-linfocito (INL) y los índices plaquetarios obtenidos del hemograma automatizado permiten predecir la gravedad clínica del dengue en adultos atendidos en el Centro de Salud I-3 Túpac Amaru de Iquitos durante 2026. Se desarrollará un estudio cuantitativo, aplicado, observacional, analítico, de cohorte retrospectiva y alcance predictivo, mediante la revisión de historias clínicas e informes de laboratorio correspondientes al periodo enero-junio de 2026. La muestra estará conformada por 94 pacientes adultos con dengue confirmado y hemograma procesado mediante el analizador Mindray BC-6000 durante las primeras 72 horas de síntomas. Se evaluarán el INL, volumen plaquetario medio (VPM), ancho de distribución plaquetario (PDW), proporción de plaquetas grandes (P-LCR) y plaquetocrito (PCT), relacionándolos con la gravedad clínica según la clasificación de la OMS. Se aplicarán análisis descriptivos, prueba U de Mann Whitney, curvas ROC, área bajo la curva, índice de Youden y regresión logística binaria. Se espera identificar indicadores hematológicos útiles para la predicción temprana de la gravedad del dengue en el contexto local.

Dengue poses a significant public health problem in the Peruvian Amazon region, where the early identification of patients at risk of progressing to severe disease presents a challenge for primary care services. This project aims to evaluate whether the neutrophil-to-lymphocyte ratio (NLR) and platelet indices derived from automated complete blood counts (CBC) can predict the clinical severity of dengue in adults treated at the Túpac Amaru I-3 Health Center in Iquitos during 2026. A quantitative, applied, observational, analytical, retrospective cohort study with a predictive scope will be conducted by reviewing medical records and laboratory reports from the January–June 2026 period. The sample will consist of 94 adult patients with confirmed dengue who underwent CBC analysis using a Mindray BC-6000 analyzer within the first 72 hours of symptom onset. The study will evaluate the NLR, mean platelet volume (MPV), platelet distribution width (PDW), large cell ratio (P-LCR), and plateletcrit (PCT), correlating them with clinical severity based on WHO classification. Statistical methods will include descriptive analysis, the Mann-Whitney U test, ROC curves, area under the curve (AUC) analysis, the Youden index, and binary logistic regression. The study expects to identify hematological indicators useful for the early prediction of dengue severity within the local context.
