Effectiveness of simple and modified Valsalva maneuvers in supraventricular tachycardia

Abstract

Objective: Systematize the evidence on the Efficacy of the Simple and Modified Valsalva Maneuver in Supraventricular Tachycardia. Materials and Methods: The type of research is quantitative, the study design is a systematic review, the population was 45 trials, and the sample was 10 scientific trials endorsed and described in the scientific database: SciElo, Dialnet, World wide Science, Google Scholar, Sholarpedia, Academia.edu, Springer Link, Refseek, CERN Document Server, Microsoft Academic, JURN, BASE, ERIC, ScienceResearch.com, iSEEK Education, and PubMed; To estimate the trials, the GRADE method was used, which allowed estimating the quality of evidence and level of recommendation. Once the systematic review of the trials has been completed, of the 100%, 20% correspond to the USA, Canada and England and 10% to Cuba, Australia, Turkey and Austria, respectively. Regarding the design and type of study, 70% belong to Clinical Trials, 20% to meta-analysis and 10% to systematic review. Results: Subsequent to the systemic review, it is resolved that 90% of the publications related to the Simple and Modified Valsalva Maneuver attribute them as effective procedures because they showed reversal to sinus rhythm after its application and 10% conclude that they are not effective, because there was no evidence of a decrease in heart rate after the application of the maneuver, which was insufficient to support its therapeutic use. Conclusions: It is recommended that health personnel extend the application of the Simple and Modified Valsalva Maneuver within the clinical practice in the treatment guidelines for Supraventricular Tachycardia as a first-choice therapy because it is an effective intervention and with fewer adverse events. than regular pharmacological therapy.

Objetivo: Sistematizar las evidencias sobre la Eficacia de la Maniobra de Valsalva Simple y Modificada en Taquicardia Supraventricular. Materiales y Métodos: El tipo de investigación es cuantitativa, el diseño de estudio es revisión sistemática, la población fue de 45 ensayos, y la muestra fue de 10 ensayos científicos endozados y descritos en la base de datos científicos: SciElo, Dialnet, World wide Science, Google Scholar, Sholarpedia, Academia.edu, Springer Link, Refseek, CERN Document Server, Microsoft Academic, JURN, BASE, ERIC, ScienceResearch.com, iSEEK Education, y PubMed; para la estimación de los ensayos se utilizó el método GRADE el cual permitio estimar la calidad de evidencia y nivel recomendación. Culminada la revisión sistemática de los ensayos, del 100%, el 20% corresponden a EE.UU, Canada e Inglaterra y el 10% a Cuba, Australia, Turquia y Austria, respectivamente. En lo referido al diseño y tipo de estudio el 70% pertenece a Ensayos Clínicos, 20% a meta-análisis y 10% a revisión sistemática. Resultados: Subsecuente a la revisión sistémica, se resuelve que el 90% de los publicaciones relacionadas a la Maniobra de Valsalva Simple y Modificada atribuyen como procedimientos eficaces debido a que mostraron reversión a ritmo sinusal posterior a su aplicación y el 10% concluye que no son eficaces, porque no se evidenció disminución del ritmo cardiaco posterior a la aplicación de la maniobra siendo insuficiente para respaldar su uso terapeutico. Conclusiones: Se recomienda al personal de salud, hacer extensiva dentro de la practica clínica la aplicación de la Maniobra de Valsalva Simple y Modificada en las guías de tratamiento para la Taquicardia Supraventricular como terapia de primera elección por ser una intervencion eficaz y con menos eventos adversos que una terapia farmacologica habitual.
