Effectiveness of albumin treatment versus other fluids in patients with sepsis

Abstract

Objective: To analyze and systematize the evidence on the effectiveness of albumin treatment versus other fluids in patients with sepsis. Materials and Methods: The design is a systematic review, using the GRADE system. The search was conducted in the following databases: Cochrane, Web of Science, OvidSP, PubMed, clinicaltrials.gov, controlled-trials.com, Medline, Elsevier, Web of Science, Embase. The population consisted of 10 articles, including systematic reviews and study samples. Four studies are clinical trials, three are systematic reviews and meta-analyses, and three are meta-analyses only. Results: Of all the studies, 60% demonstrate the effectiveness of albumin as an intravascular volume expander, decreasing the risk of renal failure, pulmonary edema levels, and significantly reducing extravascular pulmonary water. 30% demonstrate the effectiveness of other fluids as the first line in fluid resuscitation due to their market accessibility. 10% demonstrate the effectiveness of both albumin and other fluids in increasing left ventricular stroke work and systolic volume. Conclusions: Of the 10 articles, 6 demonstrate the effectiveness of albumin in patients with sepsis by reducing mortality rates, 3 demonstrate that other fluids are more effective than albumin because it showed no change in mortality, and 1 demonstrates that both albumin and other fluids are effective in increasing cardiac index.

Objetivo : Analizar y sistematizar las evidencias sobre la eficacia del tratamiento con albúmina versus otros fluidos en pacientes con sepsis. Materiales y Métodos : El diseño es revisión sistemática, se utilizó el sistema GRADE, se realizó la búsqueda en las siguientes bases de datos: Cochrane, Web of Science, OvidSP, PubMed, clinicaltrials.gov, controlled-trials.com, Medline, ElSevier, Web of Science databases, Embase , la población fue de 10 artículos entre revisiones sistemáticas y la muestra de 10 artículos de estudio. 4 estudios corresponden a ensayos clínicos, 3 a revisiones sistemáticas y meta- análisis, y otros 3 solo a meta-análisis. Resultados : De todos los estudios, el 60% evidencia la eficacia de la albúmina por ser expansor del volumen intravascular, disminuir el riesgo de falla renal, los grados de edema pulmonar y significativamente de agua pulmonar extravascular , 30%evidencia la eficacia de otros fluidos por ser la primera línea en la reanimación con líquidos, por su fácil accesibilidad al mercado y un 10% evidencia la eficacia de albúmina y otros fluidos por incrementar el trabajo del ictus ventricular izquierdo, el volumen sistólico. Conclusiones : De 10, en 6 artículos se evidencia la eficacia de la albúmina en pacientes con sepsis porque disminuye las tasas de mortalidad, en 3 se evidencia que otros fluidos son más eficaces que la albúmina porque esta no presento cambios en la mortalidad, y en 1 se evidencia que tanto la albúmina como otros fluidos son eficaces por incrementar el índice cardiaco.
