Effectiveness of the Pap smear compared to other tests for early diagnosis of cervical cancer in women of reproductive age

Abstract

Materials and Methods: Systematic Review that synthesizes the results of multiple investigations, identifying the most relevant to answer specific questions of daily practice. The search was restricted to articles with full text subjected to critical reading, using the GRADE system to assign the strength of recommendation. In the final selection, 10 articles were chosen, finding that 40% (04) correspond to Bolivia, Colombia, Cuba, Peru, 30% (03) to Belgium, Greece, Spain, 20% (02) to the USA and 10% ( 10) to Asia. Of the Studies, 30% correspond to Systematic Reviews, 30% Descriptive correlational type, 20% Randomized Clinical Trials and 20% Cases and Controls. Results: 50% of the articles reviewed consider the need to complement the PAP with the Colposcopy exam as diagnostic support, while 30% mention that the PAP should be accompanied by the HPV test and 20% affirm that the Pap smear It is the best test for early detection of cervical cancer. Conclusions: In 5 of the 10 articles reviewed on the effectiveness of PAP compared to other examinations, it is evident that colposcopy is a complementary examination in the definitive diagnosis of cervical cancer in women of childbearing age.

Materiales Y Métodos: Revisión Sistemática que sintetiza los resultados de múltiples investigaciones, identificando lo más relevante para responder preguntas específicas de la práctica diaria. La búsqueda fue restringida a artículos con texto completo sometidos a una lectura crítica, utilizando el sistema GRADE para asignar la fuerza de recomendación. En la selección definitiva se eligieron 10 artículos, encontrando que el 40% (04) corresponde a Bolivia, Colombia, Cuba, Perú, 30% (03) a Bélgica, Grecia, España, 20% (02) a USA y 10% (10) a Asia. De los Estudios el 30% corresponde a Revisiones Sistemáticas, 30% Descriptivos de tipo correlacional, 20% Ensayos Clínicos Aleatorizados y 20% a Casos y Controles. Resultados: El 50% de los artículos revisados considera la necesidad de complementar al PAP con el examen de Colposcopía como apoyo de diagnóstico, mientras que el 30% menciona que el PAP debe acompañarse con el examen de VPH y un 20% afirman que el Papanicolaou es el mejor examen para la detección temprana del cáncer cervical. Conclusiones: En 5 de los 10 artículos revisados sobre la efectividad del PAP comparado con otros exámenes se evidencia que la colposcopia es un examen complementario en el diagnóstico definitivo del cáncer del cuello uterino en mujeres en edad fértil.
