Critical review: effect of dietary fiber intake on uric acid and creatinine levels in adult patients on hemodialysis

Abstract

Although hemodialysis successfully prolongs and improves the quality of life for renal patients, fiber intake also affects this process by reducing toxins (urea, creatinine, etc.). The clinical question was: What is the effect of dietary fiber intake on uric acid and creatinine levels in adult patients on hemodialysis? Evidence-based nutrition (NuBE) methodology was used. The search for information was conducted in PUBMED, MEDLINE, SCIELO, SCOPUS, and SCIENCE DIRECT, yielding 22 articles, 9 of which were selected for evaluation using the CASPE tool. The randomized clinical trial titled "Effects of Fermentable High Fiber Diet Supplementation on Gut-Derived and Conventional Nitrogenous Products in Maintenance Hemodialysis Patients" was chosen, with a level of evidence AI and a strong recommendation grade. The critical commentary concluded that a diet enriched with fermentable fiber is suitable for maintenance dialysis patients due to its safety and absence of significant side effects, and it significantly reduces creatinine and uric acid levels.

Aunque la hemodiálisis prolonga y mejora con éxito la calidad de vida de los pacientes renales, la fibra también afecta a este nivel debido a la reducción de toxinas (urea, creatinina, etc.). La pregunta clínica fue: ¿Cuál es el efecto de la ingesta fibra dietaria en los niveles de ácido úrico y creatinina de pacientes adultos en hemodiálisis? Se utilizó la metodología Nutrición Basada en Evidencia (NuBE). La búsqueda de información se realizó en PUBMED, MEDLINE, SCIELO, SCOPUS, SCIENCE DIRECT, encontrando 22 artículos, siendo seleccionados 9 que han sido evaluados por la herramienta para lectura crítica CASPE, seleccionándose finalmente el Ensayo Clínico Aleatorizado titulado como “Effects of fermentable high fiber diet supplementation on gut derived and conventional nitrogenous product in patients on maintenance hemodialysis ”, el cual posee un nivel de evidencia AI y Grado de Recomendación Fuerte, de acuerdo a la expertos del investigador. El comentario crítico permitió concluir que la administración de una dieta enriquecida con fibra fermentable es adecuada para pacientes en diálisis de mantenimiento por su seguridad y ausencia de efectos secundarios importantes y reduce de manera significativa los niveles de creatinina y ácido úrico.
