Factors associated with therapeutic adherence in patients with hypertension in the patients with arterial hypertension at the Buenos Aires de Villa health center in the year 2023

Abstract

Introduction: Chronic diseases remain a growing problem. Treatment includes lifestyle changes, pharmacological treatment, physical activity, among others. These changes require therapeutic adherence from patients. Hypertension is currently one of the most prevalent chronic diseases, with elevated blood pressure representing two-thirds and half of all cases of stroke and ischemic heart disease, respectively. Objective: To determine the factors associated with therapeutic adherence in patients with hypertension at the Buenos Aires de Villa Health Center in 2023. Methodology: Cross-sectional analytical study. The Morisky-Green instrument was used to evaluate adherence to pharmacological treatment in hypertensive patients at the Buenos Aires de Villa Health Center in Lima during 2023. Sociodemographic and clinical variables related to hypertension treatment were measured. A bivariate analysis was performed to determine the association between the studied variables. Results: It was found that 58.0% of patients had adherence to treatment, and 132 (42.0%) did not. Sociodemographic factors associated with treatment adherence were marital status (p=0.001) and living alone (p<0.001). Clinical factors associated with treatment adherence were having a comorbidity (p<0.001). Conclusion: Treatment adherence in hypertensive patients at the Buenos Aires de Villa Health Center was high; and the factors associated with treatment adherence were marital status, living alone, and having a comorbidity.

Introducción: Las enfermedades crónicas siguen siendo una problemática que va en aumento. El tratamiento incluye cambios en el estilo de vida, tratamiento farmacológico, actividad física, ente otros. Estos cambios requieren de adherencia terapéutica de los pacientes. La hipertensión arterial actualmente una de las enfermedades crónicas con más prevalencia, actualmente la presión arterial elevada representa dos tercios y la mitad de todos los casos de accidente cerebrovascular y cardiopatía isquémica. Objetivo: Determinar los factores asociados a la adherencia terapéutica en pacientes con hipertensión arterial del centro de salud Buenos Aires de Villa en el año 2023. Metodología: Estudio transversal analítico. Se empleó el instrumento Morisky-Green para evaluar la adherencia al tratamiento farmacológico en pacientes hipertensos del centro de salud Buenos Aires de Villa, en Lima durante el 2023. Se midieron variables sociodemográficas y clínicas relacionadas al tratamiento de hipertensión arterial. Se realizó un análisis bivariado y determinar la asociación entre las variables estudiadas. Resultados: Se encontró que 58.0% de pacientes sí tuvieron adherencia al tratamiento y 132 (42.0%) pacientes no tuvieron adherencia al tratamiento. Los factores sociodemográficos asociados a la adherencia al tratamiento fueron el estado civil (p=0.001) y vivir solo (p<0.001). Los factores clínicos asociados a la adherencia al tratamiento fueron tener una comorbilidad (p<0.001). Conclusión: La adherencia al tratamiento en pacientes hipertensos del centro de salud Buenos Aires fue alta; y los factores asociados a la adherencia al tratamiento fueron el estado civil, vivir solo, tener una comorbilidad.
