Efficacy of the Harmonic Scalpel versus the Electronic Scalpel in patients undergoing surgery

Abstract

Objective: To analyze and systematize the evidence found on the effectiveness of the harmonic scalpel versus the electronic scalpel in patients undergoing surgery. Materials and Methods: The study was quantitative, and the study design was a systematic review. The instrument used was a database search: PubMed, Scielo, Epistemonikos, and EBSCO, with a population of 15 articles, and the sample consisted of 10 scientific articles published in scientific databases. The GRADE system was used to evaluate the selected articles. Results: Ten scientific articles on the effectiveness of the harmonic scalpel versus the electronic scalpel in patients undergoing surgery were reviewed. The systematic review of the articles, from a total of 100%, included 10% from Canada, 10% from Taiwan, 10% from Italy, 10% from China, 10% from Iraq, 10% from Spain, 10% from Brazil, 10% from Turkey, and 20% from India. Regarding the designs and studies, 10% were retrospective, 10% were randomized prospective trials, 10% were clinical trials, 30% were meta-analyses, and 40% were random cases. On the other hand, it was found that the use of the harmonic scalpel is more effective because it reduced the operative time, postoperative pain, intraoperative bleeding, and the patient's hospital stay. Conclusions: The use of the harmonic scalpel is 100% more effective than the electronic scalpel, but economically, there are limitations regarding the studies on the harmonic scalpel, which calls for the need for further studies.

Materiales y Métodos: El tipo de estudio fue cuantitativo, el diseño del estudio fue revisión sistemática, el instrumento fue búsqueda de base de datos: PubMed, Scielo, Epistemonikos y EBSCO, con una población de 15 artículos y la muestra es de 10 artículos científicos publicados en las bases de datos científicos. Para la evaluación de los artículos seleccionados se utilizó el Sistema GRADE. Resultados: Se revisaron 10 artículos científicos sobre eficacia del bisturí armónico versus bisturí electrónico en pacientes sometidos a cirugía. La revisión sistemática de los artículos, del 100%; un 10% corresponden a Canadá, un 10% Taiwán, un 10% Italia, un 10% china, un 10% Irak, un 10% España, un 10% Brasil, un 10% Turquía y un 20% India. En relación de los diseños y estudios fueron un 10% retrospectivo, 10% Ensayo prospectivo aleatorizado, 10% Ensayo clínico, 30% meta-análisis, 40 % caso aleatorio. Por otro lado, se evidencia que el uso del bisturí armónico es más eficaz porque reduce el tiempo operatorio, el dolor post operatorio, reduce el sangrado intra operatorio y la estancia hospitalaria del paciente. Conclusiones: El uso del bisturí armónico es eficaz en 100% que el bisturí electrónico, pero en el ámbito económico hay limitaciones en cuanto los estudios del bisturí armónico encontrados, lo cual conlleva a la necesidad de analizar más estudios adicionales.
