Presence of Titillating Cells in Urinary Sediment and Its Relationship with the Diagnosis of Acute Pyelonephritis in Patients Attended at the Urology Service of Daniel Alcides Carrión National Hospital in May – June 2017

Abstract

Acute pyelonephritis, considered the third most important bacterial infection statistically, is the most significant disease in renal pathology. The Sternheimer and Malbin staining technique provided a simple and accessible means for diagnosing pyelonephritis, where titillating cells—polymorphonuclear leukocytes containing cytoplasmic granules that exhibit Brownian motion—can be observed. The objective of the research was to determine if there is a direct relationship between the presence of titillating cells in urine sediment and the diagnosis of acute pyelonephritis. A descriptive, prospective, cross-sectional study was conducted. Samples were obtained from patients in the Urology service at the Daniel Alcides Carrión National Hospital. A total of 206 samples were analyzed using Sternheimer-Malbin staining. Forty-seven samples tested positive for titillating cells, and 32 of these (100%) were related to acute pyelonephritis. Based on the results of titillating cells and acute pyelonephritis, the Chi-square test was performed to assess the association between these two variables, yielding a p-value < 0.05 and a 95% confidence level, concluding that there is an association between both results.

La pielonefritis aguda considerada como la tercera infección bacteriana, estadísticamente, es el padecimiento más importante en patología renal. Por ello la técnica de coloración de Sternheimer y Malbin aporto un medio sencillo y accesible para el diagnóstico de la pielonefritis en donde se puede observar a las células titilantes denominadas a los leucocitos polimorfonucleares que contienen gránulos citoplasmáticos que exhiben movimiento browniano. El objetivo de la investigación es determinar si existe una relación directa entre la presencia de células titilantes en sedimento urinario y el diagnóstico de pielonefritis aguda. Se realizó un estudio descriptivo, prospectivo, transversal. Se obtuvieron las muestras del Hospital Nacional Daniel Alcides Carrión de los pacientes del servicio de Urología. Las 206 muestras fueron analizadas mediante la coloración de Sternheimer Malbin. Se obtuvo un total de 47 muestras positivas a células titilantes del cual un total de 32 muestras (100%) tienen relación entre pielonefritis aguda y células titilantes. En base a los resultados de células titilantes y pielonefritis aguda, se realizó la prueba de Chi cuadrado en búsqueda de asociación entre estas dos variables, con un valor de p < 0.05 y un nivel de confianza del 95%, concluyendo así que existe asociación entre ambos resultados.
