Critical review: nutritional management in patients with neoplasia of the head and neck undergoing chemo and/or radiotherapy treatment

Abstract

Nutritional management is important for oncological patients as it helps keep them stable during chemotherapy and/or radiotherapy treatment. This secondary research titled: ‘CRITICAL REVIEW: NUTRITIONAL MANAGEMENT IN PATIENTS WITH HEAD AND NECK NEOPLASIA UNDER CHEMOTHERAPY OR RADIOTHERAPY TREATMENT’ aimed to determine the best nutritional management for patients with head and neck neoplasia undergoing chemotherapy or radiotherapy treatment, identify the common problems that occur before, during, and after chemotherapy and radiotherapy treatment concerning the nutritional status of cancer patients. The clinical question was: ‘What is the nutritional management for patients with head and neck neoplasia undergoing chemotherapy or radiotherapy treatment?’ The Evidence-Based Nutrition (EBN) methodology was used. Information was gathered from PubMed/MEDLINE, ScienceDirect, WOS, and SCOPUS, yielding 20 articles, with the final selection based on the ‘CASPE’ tool from the meta-analysis titled: 'Nutrition management for patients with head and neck cancer during peri-radiotherapy: A systematic review and quality appraisal of clinical practice guidelines using the AGREE II instrument.' The critical commentary concluded that there were discrepancies in existing clinical practice guidelines (CPGs), including nutritional screening, assessment, intake, nutrients, the absence of essential parts of the CPGs, the statement of external review, the method for making recommendations, strategies for improving acceptance, and the implications of resource use in applying the CPGs. Specialized CPGs should be developed for the nutritional treatment of head and neck cancer patients during radiotherapy.

El manejo nutricional es importante en los pacientes oncológicos ya que ayudará a mantenerlos estables durante el tratamiento con quimioterapia y / o radioterapia. La presente investigación secundaria titulada: “REVISIÓN CRÍTICA: MANEJO NUTRICIONAL EN PACIENTES CON NEOPLASIA DE CABEZA Y CUELLO CON TRATAMIENTO QUIMIO O RADIOTERAPIA”, tuvo como objetivo conocer el mejor manejo del estado nutricional de pacientes con neoplasia de cabeza y cuello con tratamiento quimio o radio terapia , identificar los problemas frecuentes que se dan antes , durante y después del tratamiento con quimioterapia y radioterapia en relación con el estado nutricional de los pacientes con cáncer. La pregunta clínica fue: “¿Cuál es el manejo nutricional en pacientes con neoplasia de cabeza y cuello con tratamiento quimio o radioterapia?”. Se utilizó la metodología Nutrición Basada en Evidencia (NuBE). La búsqueda de información se realizó en Pubmed/MEDLINE, Science direct, WOS y SCOPUS”, encontrando 20 artículos, seleccionando al final del “CASPE” el Meta análisis titulado: “ “Nutrition management for patients with head and neck cancer during peri radiotherapy: A systematic review and quality appraisal of clinical practice guidelines using the AGREE II instrument” El comentario crítico permitió concluir que encontraron recomendaciones discrepantes en las GPC existentes, incluido el cribado nutricional, la evaluación nutricional, la ingesta nutricional, los nutrientes, la ausencia de partes esenciales de las GPC y las opiniones de sus usuarios objetivo, la declaración de revisión externa, el método para formular las recomendaciones, las estrategias para mejorar la aceptación y las implicaciones de recursos de la aplicación de las GPC. Deberían elaborarse GPC especializadas sobre el tratamiento nutricional de los pacientes con cáncer de cabeza y cuello (CCC) durante la radioterapia.
