Frequent errors of care annotations in nursing records.

Abstract

Objective: To analyze and systematize the evidence on common errors in nursing care documentation. Materials and Methods: A systematic review of 13 scientific articles was conducted, using the following databases: BVS, Cochrane Library, LILACS, Scielo, Google Scholar, Pubmed, Espistemonikos. The search for articles was restricted to full text, and they were critically reviewed according to their design type. Of the articles reviewed, 84.5% were descriptive, 7.6% systematic, and 7.6% quasi-experimental. Results: Of the total of 13 articles reviewed, 100% (n=13/13) show that the main failures are the lack of identification and stamps, absence of some important items when filling them out, illegible handwriting, spelling errors, use of incorrect terminology, use of correction fluid, and a large percentage are incomplete in terms of their structure and context, and lack of information about the care provided. Conclusions: In 13 of the 13 studies, the existence of frequent errors in nursing care documentation is demonstrated. Additionally, the reviewed studies highlighted that common errors included lack of personal identification, illegibility, non-standardized terminology, absence of stamps and signatures, presence of drafts and correction fluid, among others.

Objetivo: Analizar y sistematizar las evidencias sobre los errores frecuentes de las anotaciones de los cuidados en los registros de enfermería. Materiales y Métodos: Revisión sistemática de 13 artículos científicos, se utilizó la base de datos: BVS, Cochrane Library, LILACS, Scielo, Google Académico, Pubmed, Espistemonikos. La búsqueda de artículos se restringió a texto completo y se sometieron a una lectura crítica, según el tipo de diseño, el 84.5% es descriptivo, el 7,6% sistemática y el 7,6% cuasi - experimental. Resultados: Del total de 13 artículos revisados, el 100% (n=13/13) muestran, que las principales fallas son la falta de identificación y sellos, ausencia de algunos ítems importante al llenarlos, letras ilegibles, errores de ortografía, uso de terminología incorrecta, uso de correctores, en un gran porcentaje se encuentran incompletas en cuanto a su estructura y contexto, falta de información acerca a la atención brindada. Conclusiones: En 13 de las 13 investigaciones se demuestra la existencia de errores de manera frecuente en las anotaciones de los cuidados en los registros de enfermería. Asimismo, en las investigaciones revisadas se evidencian que los errores frecuentes fueron: falta de identificación del personal, ilegibilidad, terminología no estandarizada, ausencia de sello y firma, presencia de borradores y correctores, entre otros.
