Prevalence and factors associated with hepatic steatosis, estimated using the Fatty Liver Index in urban transport drivers, Lima - 2023

Abstract

Introduction: Hepatic steatosis, characterized by abnormal fat accumulation in the liver, has emerged as a significant public health concern. This phenomenon is not only linked to various medical conditions, but a higher prevalence has also been observed in specific populations. In this context, the objective of this study is to investigate the prevalence and associated factors of hepatic steatosis in a particularly vulnerable group: urban transport workers. Methods: This is an observational, cross-sectional study where the Fatty Liver Index was calculated for 50 urban transport workers, and the relationship with factors that could increase the risk of the condition was determined. The Chi-square statistic or Fisher’s exact test was used, as appropriate. The binary logistic regression test was employed to calculate the odds ratio. Results: Hepatic steatosis (fatty liver) was estimated using the Fatty Liver Index in 38 individuals out of 50 urban transport workers, resulting in a prevalence of 76%. Independent variables such as obesity (p=0.001), waist circumference (p=0.003), and a history of diabetes (p=0.005) were significantly associated with the dependent variable, hepatic steatosis, with a 11.5 times higher likelihood of the event in obese individuals, 1.59 times higher when waist circumference exceeds >88 cm in women and >102 cm in men, and 1.87 times more likely if they have a family history of diabetes. Conclusions: There was a high prevalence of hepatic steatosis in urban transport workers, and the factors of obesity, waist circumference, and a history of diabetes were significantly associated with the presence of hepatic steatosis.

Introducción: La esteatosis hepática, caracterizada por la acumulación anormal de grasa en el hígado, ha emergido como una preocupación significativa de salud pública. Este fenómeno no solo está vinculado a diversas condiciones médicas, sino que también se ha observado una mayor prevalencia en poblaciones específicas. En este contexto, el objetivo del presente estudio se propone investigar la prevalencia y los factores asociados a la esteatosis hepática en un grupo particularmente vulnerable: los transportistas urbanos. Métodos: estudio de diseño observacional, transversal, donde se calculó el Fatty Liver Index a 50 transportistas urbanos, y se determino la relación con factores que podrían aumentar el riesgo de padecer dicho evento, para lo cual se empleó el estadístico Chi cuadrado o la prueba exacta de Fisher según corresponda. Se utilizó la prueba de regresión logística binaria para el cálculo del odds ratio. Resultados: Se estimo esteatosis hepática (hígado graso) mediante el Fatty liver index en 38 individuos de un total de 50 participantes transportistas urbanos, por consiguiente, la prevalencia alcanzo el 76%. Las variables independientes: Obesidad (p= 0,001), Circunferencia de cintura (p= 0,003) y antecedentes de diabetes (p= 0,005) se asociaron significativamente a la variable dependiente esteatosis hepática, siendo 11,5 más probable de presentar el evento cuando es obeso. 1,59 más probable cuando la circunferencia de cintura es superior a > 88 cm en damas y > de 102 cm en varones, y 1,87 más probable si se tiene familiares con diagnóstico de diabetes. Conclusiones: Se presento alta prevalencia de esteatosis hepáticas en trasportistas urbanos y los fatores obesidad, circunferencia de cintura y antecedentes de diabetes se asociaron significativamente con la presencia de esteatosis hepática.
