Association between the cardiotocographic parameters of the non-stressful test and pre-eclampsia in pregnant women treated at the San José Hospital, Callao, 2016 and 2017

Abstract

An observational, retrospective, and cross-sectional study was conducted, reviewing 118 medical records of pregnant women with preeclampsia who attended the Fetal Medicine Unit at San José Hospital, Callao, for a non-stress test (NST). The results showed that 80.5% of the pregnant women had preeclampsia without severe criteria, and 19.5% had severe criteria. Baseline fetal heart rates of 120-160 bpm were observed in 97.5% of cases, 72.9% of variability ranged from 5-9 or >25 bpm, 72.9% of accelerations were ≥5 bpm, 87.3% showed absent decelerations, and 96.6% had fetal movements ≥5. Only variability showed slight alteration as it did not fall within the normal range. NST reactivity was present in 95.8% of cases, 3.4% were non-reactive, and 0.8% were pathological. A statistically significant association was established between fetal movements as the main CTG parameter of the NST and preeclampsia without severe criteria, with a significance level of 0.013 < 0.05. NST results also showed a significance level of 0.034 < 0.05, statistically associated with preeclampsia without severe criteria. In conclusion, the parameter affected by preeclampsia without severe criteria is variability, while fetal movements and NST results demonstrate statistical association.

Se realizó un estudio observacional, retrospectivo y transversal en el que se revisaron 118 historias clínicas de gestantes con preeclampsia que acudieron a la unidad de Medicina Fetal del Hospital San José, Callao, para realizarse el test no estresante. Los resultados mostraron que el 80.5 % de las gestantes correspondían a preeclampsia sin criterios de severidad y el 19.5 % con criterios de severidad. El 97.5 % correspondió a líneas de base de 120-160 lpm, el 72.9 % de variabilidad fue de 5-9 o >25 lpm, el 72.9 % de aceleraciones mostraron ≥5 lpm, el 87.3 % presentó desaceleraciones ausentes y el 96.6 % de movimientos fetales fueron ≥5. Únicamente la variabilidad mostró ligera alteración al no encontrarse dentro del valor normal. El 95.8 % del test no estresante presentó reactividad, el 3.4 % no fue reactivo y el 0.8 % fue patológico. Se estableció una asociación estadísticamente significativa con los movimientos fetales como principal parámetro CTG del test no estresante y la preeclampsia sin criterios de severidad, con un nivel de significancia de 0.013 < 0.05. Para los resultados del NST, el nivel de significancia fue de 0.034 < 0.05, asociándose estadísticamente también con preeclampsia sin criterio de severidad. En conclusión, el parámetro alterado por la preeclampsia sin criterio de severidad es la variabilidad, y los que muestran asociación estadística son los movimientos fetales, seguidos de los resultados del test no estresante.
