Placent acretism in manager of high risk obstetric with previous placenta diagnosis in EESS de level III – february 2018

Abstract

Objective: To describe the management of a case of placenta accreta in a patient diagnosed with placenta previa at a level III healthcare facility. Materials and Methods: A descriptive, observational, retrospective study based on the review of the clinical history of a patient with placenta accreta treated in February 2018. Results: A 37-year-old multigravida patient at 33 weeks of gestation had three prenatal checkups and an incomplete perinatal card. She presented with vaginal bleeding and was referred to a higher-level healthcare facility during the third trimester of pregnancy. The diagnosis was total placenta previa with placenta accreta. The pregnancy was managed by hospitalization, but due to delays in determining the degree of accreta, the pregnancy was terminated by cesarean section and hysterectomy. A live male newborn was delivered, weighing 3010 grams, with an APGAR score of 6 at 1 minute and 7 at 5 minutes. Following the hysterectomy, the patient experienced hypovolemic hemorrhagic shock due to placenta accreta. Blood products were transfused, and she was stabilized. The patient had a favorable recovery and was discharged on the third day post-cesarean. Conclusion: The patient and her newborn had favorable outcomes despite the risk factors and complications encountered.

Objetivo: Describir el manejo de un caso de acretismo placentario en una paciente con diagnóstico de placenta previa en un establecimiento de salud de nivel III. Materiales y Métodos: estudio descriptivo, observacional, retrospectivo de revisión de historia clínica de paciente con acretismo placentario, atendida en febrero del 2018. Resultados: Paciente gran multigesta de 37 años, y 33 semanas de gestación, tuvo 3 controles prenatales; un carnet perinatal con datos incompletos. Presentó sangrado vaginal y es referida a un establecimiento de mayor nivel, durante el tercer trimestre de embarazo. Su diagnóstico fue placenta previa total más acretismo placentario. Fue hospitalizada y se trató de determinar el grado de acretismo pero ante la demora del examen, se culminó del embarazo por cesárea más histerectomía, obteniéndose un recién nacido vivo de sexo masculino, peso de 3010 gr., APGAR 6’ y 75’. Tras la histerectomía se produce un shock hipovolémico hemorrágico por incretismo placentario, se transfunde hemoderivados y es estabilizada. Tuvo una evolución favorable, siendo dada de alta al tercer día posterior a la cesárea. Conclusión: Se concluyó con una paciente de evolución favorable y su recién nacido vivo, a pesar de todos los factores de riesgo y complicaciones presentados.
