Diagnostic performance of hematological indices in the indentification of severity and mortality of patients with dengue, Hospital II-2 Tarapoto, periodo 2021-2023

Abstract

Dengue is a febrile viral disease transmitted by the bite of Aedes aegypti mosquitoes. It is one of the most common tropical diseases and a serious public health problem worldwide. Patients with dengue begin as asymptomatic until they show signs such as high fever, general discomfort, rash on upper and lower limbs, and it can progress to severe hemorrhagic fever and shock syndrome if not detected and treated timely. Diagnosis is made through laboratory tests using screening techniques such as rapid immunochromatographic methods, ELISA to detect IgM, IgG, NS1 antigen, and molecular methods like PCR for confirmation. However, the effectiveness of treatment and monitoring of the disease is subject to periodic laboratory test evaluations, including a complete blood count, where parameters tend to be altered, such as thrombocytopenia, leukopenia, presence of atypical lymphocytes, elevated hematocrit, and hemoglobin concentration. It also allows for calculating indices to evaluate the systemic inflammatory response to different diseases.

El dengue es una enfermedad viral febril transmitida por la picadura de Aedes aegypti, es una de las patologías tropicales más comunes y un serio problema de salud pública a nivel mundial, los pacientes con dengue empiezan siendo asintomáticos hasta dar señales de fiebres altas, malestar general, erupciones en miembros superiores e inferiores y puede evolucionar a una fiebre hemorrágica grave y síndrome de shock si no se detecta y da tratamiento oportuno. El diagnóstico se realiza por laboratorio con pruebas de cribado como técnicas rápidas inmunocromatográficas, ELISA que detectan Ig M, Ig G, antígeno NS1; y moleculares como PCR para confirmación. Sin embargo, la efectividad del tratamiento y monitoreo de la enfermedad está sujeta a la evaluación de controles periódicos por pruebas de laboratorio, entre ellos el hemograma, cuyos parámetros tienden a alterarse, como trombocitopenia, leucopenia, presencia de linfocitos atípicos, incremento del hematocrito y la concentración de la hemoglobina; así mismo permite calcular índices que permiten evaluar la respuesta inflamatoria sistémica ante diferentes enfermedades.
