Effectiveness of ibuprofen combined with acetaminophen for controlling fever in children under 5 years old

Abstract

Objective: To determine the effectiveness of Ibuprofen combined with paracetamol for controlling fever in children under 5 years old. Materials and Methods: This is a systematic review that included 7 articles, no older than 10 years, with full-text publications in English, Portuguese, and Spanish, from the databases: PubMed, Scielo, Journal Medical, Cochrane Plus, and BVS. These articles focused on the effectiveness of Ibuprofen combined with paracetamol for fever control in children under 5 years old. 42.8% (3) of the articles are from South American countries, including Peru, while 14.2% (1) each are from Spain, Canada, Mexico, and Nigeria. Results: Of the total articles analyzed, 71.4% recommend Ibuprofen as the treatment of choice for significantly reducing fever. Additionally, when used in combination with Paracetamol or Acetaminophen, it not only increases effectiveness in lowering the temperature but also extends the duration the child remains afebrile. Conclusions: The evidence shows that the combined therapy of Ibuprofen and Paracetamol is more effective for treating fever in children than monotherapy. However, it is important to note that the use of Ibuprofen should be limited to the initial treatment due to its adverse effects. Furthermore, it is evident that Paracetamol, when combined with physical methods, significantly reduces body temperature within the first hour, more effectively than physical methods alone.

Objetivo: Determinar la eficacia del Ibuprofeno combinado con paracetamol para el control de la fiebre en niños menores de 5 años. Materiales y Métodos: Es una revisión sistémica que incluyó 07 artículos con una antigüedad no mayor de 10 años, con textos completos publicados en inglés, portugués y español en la base de datos: Pubmed, Scielo, Journal Medical, Cochrane Plus y Bvs que tuvieron como tema principal la eficacia del Ibuprofeno combinado con paracetamol para el control de la fiebre en niños menores de 5 años. El 42.8% (03) corresponden a países de Sudamérica entre ellos Perú mientras que con un 14.2% (01) encontramos a España, Canadá, México y Nigeria cada uno respectivamente. Resultados: Del total de artículos analizados, el 71.4% coloca al Ibuprofeno como tratamiento de elección que desciende significativamente la fiebre; y que en terapia conjunta con el Paracetamol o Acetaminofén además de lograr mayor eficacia descendiendo la temperatura produce también incremento del tiempo que el niño permanecerá afebril. Conclusiones: Las evidencias demuestran que la terapia combinada de Ibuprofeno con Paracetamol es más eficaz para el tratamiento de fiebre en niños, que la monoterapia; pero cabe recalcar que el uso del Ibuprofeno debiera reducirse solo al tratamiento de inicio, por los efectos adversos que presenta. También se evidencia que el paracetamol en tratamiento conjunto con los medios físicos disminuye significativamente la temperatura corporal dentro la primera hora, más que solamente el uso de medios físicos.
