Factors related to gingival melanosis in patients who attend a dental clinic Lima 2023

Abstract

The purpose of this study was to identify the factors related to gingival melanosis in patients attending a dental clinic in Lima, 2023. The study was observational, relational, cross-sectional, and prospective, where melanosis was identified according to its intensity and location based on related factors. The technique used was observation, and data were collected using a data collection sheet. Regarding the extent, the presence of at least one continuous pigmentation formed by two solitary pigmentations was noticeable both in the upper jaw (53.70%) and in the lower jaw (56.20%). Regarding intensity, most individuals exhibited mild gingival pigmentation, followed by moderate and intense pigmentation in both locations. No significant differences were found between genders (upper jaw: p=0.586, lower jaw: p=0.619), with moderate gingival pigmentation being the most common (25% in women and 27.50% in men). Regarding intensity, no significant differences were found between age groups (p=0.189), although a trend towards greater intensity was observed in the 30-59 age group in the upper jaw. Significant associations were observed in both locations, suggesting a higher presence of intense gingival pigmentation in individuals with medium and dark skin, especially in the lower jaw (p=0.002). The presence of continuous pigmentation was more pronounced in individuals with a thick phenotype and a high smile. It is concluded that there is a relationship between the factors of age, skin pigmentation, and location with the appearance of gingival melanosis in the evaluated patients.

El propósito del estudio fue identificar los factores relacionados a la melanosis gingival en pacientes que acuden a una clínica odontológica Lima 2023. El estudio fue de tipo observacional, relacional, transversal y prospectivo en donde se identificó la melanosis de acuerdo a su intensidad y ubicación de acuerdo a los factores relacionados. La técnica utilizada fue la observación, trabajando en una ficha de recolección de datos. En cuanto a la extensión, la presencia de al menos una pigmentación continúa formada por dos pigmentaciones solitarias es notoria tanto en el maxilar superior (53.70%) como en el inferior (56.20%). En relación con la intensidad, la mayoría exhibe una pigmentación gingival leve, seguida de pigmentación media e intensa en ambas ubicaciones. No se encontraron diferencias significativas entre sexos (maxilar superior: p=0.586, maxilar inferior: p=0.619), teniendo como pigmentación gingival predominante la media (25% mujeres y 27.50% varones). En cuanto a la intensidad, no se encuentran diferencias significativas entre grupos de edad (p= 0.189), aunque se observa una tendencia hacia una mayor intensidad en el grupo de 30 a 59 años en el maxilar superior. Se observan asociaciones significativas en ambas ubicaciones, sugiriendo una mayor presencia de pigmentación gingival intensa en individuos de piel Trigueña y Oscuro, especialmente en el maxilar inferior (p= 0.002), la presencia de pigmentación continua está más marcada en individuos con fenotipo Grueso y sonrisa alta. Se concluye que existe relación entre los factores edad, pigmentación de piel y ubicación con la aparición de melanosis gingival en los pacientes evaluados.
