Critical review: effect of early enteral nutrition in the postoperative period of patients with gastric cancer undergoing oncological surgery

Abstract

It is presumed that early initiation of enteral nutrition in patients with gastric cancer who have undergone gastrectomy would improve some clinical outcomes. The present secondary research entitled "Critical Review: Effect of early enteral nutrition in the postoperative period of patients with gastric cancer undergoing oncological surgery", aimed to analyze the effects (clinical outcomes and nutritional aspects) of early enteral nutrition 24 or 48 hours after oncological surgery in patients with gastric cancer. The clinical question was: Does early enteral nutrition improve clinical outcomes and nutritional aspects in patients with gastric cancer undergoing oncological surgery? The Evidence-Based Nutrition (NuBE) methodology was used. The information search was carried out in Scielo, Pubmed, Mdpi, Jboun, Ncbi, Science Direct, Hindawi, thus finding 21 articles, 10 being selected, which were evaluated by the CASPE critical reading tool, finally selecting the clinical trial titled as: “Effect of Early Enteral Nutrition Support Combined with Chemotherapy on Related Complications and Immune Function of Patients after Radical Gastrectomy”, which has a level of evidence AI and grade of recommendation strong, according to the expertise of the researcher. The critical comment allowed us to conclude that the early start of enteral nutrition could improve certain nutritional components such as total protein and blood albumin, level of immune cells, as well as total drainage through gastric tube and hospitalization time; although additional research is required to corroborate these results.

El presente trabajo es una investigación secundaria de revisión crítica, la cual tuvo como objetivo determinar el efecto de la suplementación con zinc y/o vitamina C como tratamiento para la recuperación de pacientes con COVID-19.Se utilizó como metodología la Nutrición Basada en Evidencia (NuBE), asimismo, la búsqueda sistemática de artículos se realizó utilizando como bases de datos científica a Pubmed, Science Direct, Springer Open, Scopusy MDPI, donde se encontraron 55 artículos, de los cuales 17 fueron seleccionados para ser evaluados por la herramienta CASPe y de esta manera se seleccionó un ensayo clínico aleatorizado titulado: “Effect of high-dose zinc and ascorbic acid supplementation vs usual care on symptom length and reduction among ambulatory patients with SARS-CoV-2 infection: The COVID A to Z randomized clinical trial”, el cual obtuvo un nivel de evidencia “I” y un grado de recomendación “Fuerte” en virtud a la evaluación del investigador en concordancia con la aplicación de la herramienta CASPe, concluyendo que la suplementación con zinc y/o vitamina C genera un efecto positivo en el tratamiento para la recuperación de pacientes con COVID-19, pero no es estadísticamente significativo; por lo que deben efectuarse mayores ensayos clínicos que demuestren la importancia de dicha suplementación.
