Antipyretic efficacy of combined versus individual use of acetaminophen and ibuprofen in febrile pediatric patients.
(Universidad Privada Norbert Wiener, 2018-03-25) Carlos Musayon, Jessica Viviana; Casanova Taya, Betty; Matta Solis, Hernan Hugo
Objective: To systematically analyze the evidence of the combined use of paracetamol and ibuprofen versus their individual use. Material and Method: The systematic review of 10 articles on the antipyretic efficacy of the combined use versus individual use of paracetamol and ibuprofen in febrile pediatric patients in the emergency department was found in the following databases: Cochrane, SciELO, Medline, PubMed, and Elsevier. Of the 10 articles reviewed, 60% (6/10) correspond to the methodological design of randomized clinical trials, 30% (3/10) are systematic reviews, and 10% (1/10) is a simple review. All of them were analyzed according to the GRADE scale to determine their strength and quality of evidence. Results: 80% of the reviewed articles are conclusive, supported by 1 systematic review and 1 simple review, suggesting that combined therapy is better than monotherapy, and 6 randomized clinical trials showing that the combined use of paracetamol and ibuprofen works more effectively and even faster than paracetamol or ibuprofen alone. However, the other 20% recommend that further studies in larger populations are necessary to confirm its efficacy. Conclusions: Of the 10 articles reviewed, 80% (8/10) demonstrate the efficacy of the combined use of paracetamol and ibuprofen in febrile pediatric patients.

Of these there are 2 systematic reviews, 4 meta-analyses, 3 clinical trials and 1 control study, all of high quality. Results: Of the 10 articles reviewed, 100% (10/10) demonstrated that aspiration of subglottic secretions reduces the incidence of pneumonia and edema problems compared to tracheal aspirations. Conclusions: It was shown that aspiration of subglottic secretions is effective in reducing the risk of pneumonia associated with mechanical ventilation.

Critical review: Efficacy of exercise in blood pressure control in adults.
(Universidad Privada Norbert Wiener, 2022-06-16) Olano Yalta, Anthonny Micheel; León Cáceres, Johanna del Carmen
It has been described that not only pharmacological treatment helps in the control of blood pressure, but also physical activity, especially aerobic exercises, which are one of the main effective treatments for regulating and controlling hypertension. This secondary research, titled Critical Review: Effectiveness of Exercise in Blood Pressure Control in Adults, aimed to identify whether exercises are effective in controlling blood pressure in adults. The methodology used was Evidence-Based Nutrition (NuBE). The information search was conducted in PUBMED and SCIENCE DIRECT, finding 36 articles, with 11 selected for critical evaluation using the CASPE tool. Finally, the randomized clinical trial titled Long-term home-based exercise is effective to reduce blood pressure in low-income Brazilian hypertensive patients: A controlled trial was chosen, which has an AI level of evidence and a strong recommendation grade, according to the researcher's expertise. The critical comment concluded that physical activity interventions, including low to moderate intensity walking and stretching exercises, are the only way to improve the patient's quality of life.

Of the studies, 50% are systematic reviews, 20% clinical trials, 20% meta-analysis, and 10% cohort studies. 90% of the articles are of high quality and strong strength of evidence, 10% of moderate quality and strong strength of evidence. Of the 10 studies, 40% are from the USA, 20% from Spain, 20% from Mexico, 10% from Australia, and 10% from Uruguay. Results: All the articles systematically reviewed indicate the effectiveness of remote monitoring in nursing consultations to reduce complications in patients with implantable cardiology devices. Conclusions: The studies show that remote monitoring of implantable devices in patients with heart disease is an effective, reliable and safe system that allows the detection of arrhythmias or abnormalities in the device, optimizes patient monitoring and can prevent complications such as infections, device abnormalities, among others. Likewise, they have demonstrated a large number of benefits for the monitoring and care of the patient by the health team, where the specialized nurse trained in remote monitoring consultation has a transcendental management role.
