    Effectiveness of chlorhexidine in preventing infections associated with the insertion of the hemodialysis catheter.

    Objective: This is a systematic review that included 10 articles, no older than 10 years, published in English, Portuguese, and Spanish, in the following databases: Pubmed, Scielo, Journal Medical, Lilacs, and Bvs. The main topic was the effectiveness of chlorhexidine in preventing infections associated with the insertion and use of hemodialysis catheters. 80% of the articles are from Spain, 15% from Chile and Brazil, and 5% from Valencia, respectively. Results: 100% of the selected articles demonstrate the positive effect of chlorhexidine in preventing infections associated with the insertion and use of hemodialysis catheters. 70% of the articles showed that chlorhexidine is effective in reducing bacteremia, 20% emphasized the importance of applying a set of biosafety measures to prevent infections related to central venous catheters in this patient population, and 10% focused on the application of protocols for the management of hemodialysis catheters. Conclusion: The evidence demonstrates that chlorhexidine improves the quality of life for individuals undergoing hemodialysis, as it has a positive impact on preventing bacteremia-related infections associated with hemodialysis catheters.

    Effectiveness of high-flow hemodialysis versus low-flow hemodialysis in reducing cardiovascular complications in patients with end-stage renal failure

    Objective: To systematize the evidence on the effectiveness of high-flow hemodialysis versus low-flow hemodialysis in reducing cardiovascular complications in patients with end-stage renal failure. Materials and Methods: A total of 10 studies were selected. The systematic review of the 10 scientific articles on the effectiveness of high-flow hemodialysis versus low-flow hemodialysis in reducing cardiovascular complications in patients with end-stage renal failure was conducted using the following databases: Lilacs, PubMed, Medline, and Cochrane Plus. It included 1 meta-analysis, 1 systematic review and meta-analysis, 1 systematic review, 2 experimental studies, 4 randomized clinical trials, and 1 cross-sectional study. Results: Of the 10 articles reviewed, 90% (n=9/10) agree that there is sufficient scientific evidence that high-flow hemodialysis has greater benefits than low-flow hemodialysis in preventing or delaying the onset of long-term complications in the evolutionary process of patients undergoing dialysis. 10% (n=1/10) concludes that there are no significant differences between the use of high-flow hemodialysis and low-flow hemodialysis. Conclusions: The systematically reviewed articles provide evidence that high-flow hemodialysis is more effective than low-flow hemodialysis in reducing cardiovascular complications in patients with end-stage renal failure.

    Effectiveness of kidney transplantation in obese recipients with end-stage chronic kidney disease

    Objective: To evaluate the effectiveness of kidney transplantation in obese recipients with end-stage chronic kidney disease. Materials and Methods: This is a systematic review of 10 scientific articles found in the following databases: Experimental and Clinical Transplantation, Nephrology Dialysis Transplantation, BMC Medicine, Pubmed, Journal of Renal Nutrition, Annals of Surgery, and the American Journal of Transplantation. 40% are Systematic Reviews and 60% are cohort studies. Results: 70% (n = 70/10) of the evidence indicates that obese recipients with end-stage renal disease (ESRD) have similar survival rates to non-obese patients after kidney transplantation. Additionally, 30% (n = 30/10) of the evidence suggests that obese recipients have lower survival rates compared to non-obese ESRD patients following kidney transplantation. Conclusion: Obese recipients with end-stage renal disease have similar patient and graft survival rates as non-obese patients following kidney transplantation.

    Effectiveness of kidney transplantation in the survival of patients with terminal chronic kidney failure

    The nurse specialist in nephrology care; They are part of the interdisciplinary group of transplant programs and are the ones who verify that the patient complies with the rigorous preparation at each stage of the process, playing different roles in order to achieve comprehensive care for the patient. The complexity and continuity of care at the different times and scenarios in which the transplant takes place will determine the effectiveness of the kidney transplant, increasing the survival of the patient's graft. The nurse plays a very important role, the results of the transplant will be reflected in the interventions of the nurse specialist in nephrological care, from the administration of progressive holistic care in the transplant process, as well as the educational role.

    Effectiveness of renal transplantation from a living donor vs. cadaveric donor transplantation for the survival of the graft and recipient in adult patients with terminal chronic renal disease.

    Material and Methods: A systematic search of 10 scientific articles was conducted in the following databases: Scielo, Elsevier, Pubmed, Nefrology Journal, Biomedical Journal, and Mexican and Colombian Urology Journal. The research design is as follows: 30% (3/10) systematic reviews, and 70% (7/10) cohort studies. 30% (3/10) of the evidence is of high quality, and 70% (7/10) is of moderate quality. Furthermore, 40% of the evidence comes from Spain, 20% from Cuba, 20% from Colombia, 10% from Mexico, and 10% from Chile. Results: 100% (n=10/10) of the scientific evidence consulted indicates that kidney transplantation from a living donor is more effective than kidney transplantation from a deceased donor for graft and recipient survival in adult patients with end-stage renal disease, proving to be the best treatment, as it results in fewer complications and improved quality of life. Conclusion: Kidney transplantation from a living donor is more effective than kidney transplantation from a deceased donor for graft and recipient survival in adult patients with end-stage renal disease.

    Effectiveness of the Buttonhole technique compared to the ladder technique in reducing complications from cannulation in adult patients with end-stage renal disease undergoing hemodialysis

    The systematic review was carried out in the following databases: Epistemonikos, Scielo, Pubmed, which allowed us to find 10 scientific evidence. Of which, 50% (5/10) is a systematic review type, 40% (4/10) is a randomized controlled study type and 10% (1/10) is a meta-analysis. 10% of the evidence found comes from Holland, Spain, China and the United States, 20% of the evidence comes from the United Kingdom, Australia and Canada. Results: 60% (6/10), (11, 12, 15, 18, 19 and 20), indicate that the buttonhole method is more effective than the ladder method to reduce complications due to cannulation in adult patients with chronic kidney disease. terminal on hemodialysis. 30% (3/10) of the authors (14, 16 and 17) consider that the buttonhole method is associated with complications such as local and systemic infections, pain, hematoma of the arteriovenous fistula compared to the ladder technique in patients adults on hemodialysis. 10% (1/10) of the authors (13) do not support the preferential use of the buttonhole technique over the ladder technique. Conclusion: The buttonhole technique is more effective than the ladder technique to reduce complications due to arteriovenous fistula cannulation in adult patients with end-stage chronic kidney disease on hemodialysis.

    Efficacy of dialytic treatment compared to conservative treatment for survival in elderly patients with end-stage renal disease.

    Objective: To systematize and evidence the efficacy of kidney transplantation in the reduction of the risk of graft loss for survival in pediatric patients. Materials and Methods: The study is quantitative, with a systematic review design. The population consisted of 39 articles that were reviewed, and 10 scientific articles were selected as the sample. Of these, 20% (n=2/10) were systematic reviews, 10% (n=1/10) were systematic review and meta-analysis, and 70% (n=7/10) were cohort studies. According to the quality of evidence, 30% was rated as high and 70% as moderate. In terms of the strength of recommendations, 30% were strong and 70% were weak, based on the country where the research was conducted. Results: The reviewed articles show that 40% (n=4/10) indicate the efficacy of renal replacement therapy in the risk of graft loss for survival in pediatric patients. Another 40% (n=4/10) state that it is partially effective, and 20% (n=2/10) state it is not effective. Conclusion: It is concluded that 40% (n=4/10) indicates that renal replacement therapy is partially effective in the risk of graft loss for survival in pediatric patients.

    Chronic terminal kidney disease (CTKD) is the irreversible loss of kidney function, affecting a significant portion of the global population. It represents the stage in which the patient may be in the final phase of their illness. Due to its complexity, managing this disease requires a coordinated effort to ensure that nursing professionals can provide palliative care and improve the quality of life during the final stage, preventing the outcome from being excessively painful. The primary objective of this research was to determine the relationship between nursing palliative care and the quality of life of patients with chronic terminal kidney disease at the Hospital Nacional Arzobispo Loayza in 2021.

    Revisión crítica: efecto de la suplementación con omega-3 en los marcadores de inflamación en adultos con enfermedad renal crónica terminal

    La inflamación crónica es un mal que aqueja a los pacientes con enfermedad renal crónica terminal (ERCT). Esta afecta la calidad de vida y aumenta la mortalidad de esta población. La presente investigación secundaria titulada como: ―Revisión crítica: Efecto de la suplementación con omega-3 en la inflamación en adultos con ERCT‖ tuvo como objetivo realizar un comentario crítico profesional de acuerdo a la revisión de artículos científicos de estudios clínicos sobre el tema de suplementación con omega-3 e inflamación en pacientes con ERCT. La pregunta clínica fue: ¿Cuál es el efecto de la suplementación con omega-3 en los marcadores de inflamación en adultos con ERCT? Se utilizó la metodología Nutrición Basada en Evidencia (NuBE). La búsqueda de información se realizó en Pubmed, Science direct, Scopus, HINARI y Cochrane, encontrando 21 artículos, siendo seleccionados 10 que han sido evaluados por la herramienta para lectura crítica CASPE, seleccionándose finalmente el Ensayo Clínico Aleatorizado titulado ―Effect of Omega-3 Fatty Acids Supplementation on Homocysteine Level in Patients Undergoing Continuous Ambulatory Peritoneal Dialysis‖; el cual posee un nivel de evidencia A I y Grado de Recomendación Fuerte, de acuerdo a la experticia del investigador. El comentario crítico permitió concluir que, es necesario realizar estudios más extensivos que incluyan dosis y tiempo de tratamiento adecuados de omega-3, para comprender mejor su efecto en la inflamación en pacientes con ERCT.

    Revisión crítica: impacto del ejercicio físico durante la diálisis en la calidad de vida de individuos con insuficiencia renal en fase terminal

    El ejercicio físico puede cambiar drásticamente la vida de los pacientes con insuficiencia renal terminal en diálisis. La revisión secundaria actual se titula “Revisión crítica: El ejercicio intradiálisis de baja intensidad” y su efecto sobre la calidad de vida registrada de pacientes con enfermedad renal en etapas terminales, tiene la finalidad de investigar el efecto de la actividad física. Se encuentra un ámbito clínico “¿Cuál es el efecto del ejercicio diferente en la calidad de vida para adultos que padecen insuficiencia renal en fase terminal?” y se utilizó la metodología de Nutrición Basada en Evidencias. El estudio casi integro en DynaMed1, EBSCO, BVS, Karger, PLOS-ONE, PUBMED, Springer y Wiley Online Library arrojó 37 artículos, de estos 13 fueron seleccionados para evaluar, luego se debe utilizar el instrumento CASPE para una lectura crítica y el estudio a elegir es el Randomizado “Breathing-based leg exercises during hemodialysis improve quality of life”. Es evidencia tipo “AI” y una recomendación con grado “Fuerte” por parte del IBP. Luego de este análisis se encuentra que, después de revisar y desglosar completamente el ensayo, el programa de ejercicios de la pierna baja realizado durante un periodo de 12 semanas, acompañado de los ejercicios de respiración controlada, ofrecen una forma segura y efectiva de reducir la fatiga y mejorar la calidad de vida en los adultos que reciben tratamiento de diálisis la evidencia es adecuada.
