    Ahorro económico por uso de sistema de distribución de dosis unitaria de medicamentos y material médico quirúrgico en la sala de unidad de cuidados intensivos de pacientes COVID-19 del Hospital Dos de Mayo durante el periodo 2020

    Healthcare institutions in our country do not achieve economic autonomy; therefore, they have the responsibility to manage resources in accordance with the law in a correct, timely, efficient, and economical manner, minimizing and recovering costs. This thesis aimed to determine the economic savings of the unit dose distribution system for medications and surgical medical supplies in the intensive care unit for COVID-19 patients at Dos de Mayo Hospital during the 2020 period. Methodology: The research was basic, with an observational design, a quantitative approach, and descriptive analysis based on entry notes from return forms, with 260 entry notes as the sample. The method used was deductive, and the data collection instrument was an observation sheet. Results: The savings percentages for returned medications and surgical medical supplies through the Unit Dose Distribution System were estimated as follows: 46.95% (S/.8,319.04) due to discharge, 35.79% (S/.4,307.89) from overstock reduction, 56.12% (S/.5,732.26) due to treatment changes, 13.07% (S/.82.71) from prescription errors, 39.80% (S/.7,681.76) due to deaths, and 5.11% (S/.246.57) due to adverse drug reactions (ADR), totaling an overall savings percentage of 40.74%, equivalent to S/.26,370.23. Conclusions: The study successfully analyzed the savings conditions for medications and surgical medical supplies returned through the Unit Dose Distribution System, highlighting key indicators that facilitate the interaction between pharmaceutical staff and the multidisciplinary team, including nursing, nutrition, and general medical staff, to optimize resources.

    Calidad de atención de enfermería y satisfacción familiar de los usuarios hospitalizados en la unidad de cuidados intensivos de un hospital público de Lima.

    Objetivo: Determinar la relación entre la calidad de atención de enfermería y la satisfacción familiar de los usuarios hospitalizados en la Unidad de Cuidados Intensivos de un hospital público de Lima. Material y método: La investigación responde al método hipotético deductivo, tiene enfoque cuantitativo, es aplicada, con diseño correlacional, la población de este estudio estará constituida por 33 familiares que acompañaron a los pacientes que acudieron a la UCI, de esto se obtuvo una muestra de 33, se utilizó la técnica de la encuesta y el instrumento cuestionario de calidad de atención: realizado por Palomino, Rivera y Tocto de la Universidad peruana Cayetano Heredia y el cuestionario de satisfacción de los familiares del paciente en uci: Flores y Pozo de la Universidad Nacional del Callao, que han sido sometidos por juicios de expertos, se encontró confiabilidad aplicando el coeficiente de confiablidad alfa de Cronbach obteniendo un valor de 0,832 y 0.789 respectivamente el que indicó excelente confiabilidad. Para el procesamiento de información se utilizará los programas Microsoft Excel y SPSS, con ellos se mostrarán los resultados a través de tablas y diagramas para su respectivo análisis (KS) e interpretación.

    Características clínicas, comorbilidades y mortalidad en pacientes con COVID-19 de la Unidad de Cuidados Intensivos 8 del Hospital Nacional Guillermo Almenara Irigoyen. Enero–mayo 2021. Lima 2022

    Objective: Analyze the relationship between clinical characteristics, comorbidities, and mortality in COVID-19 patients in ICU 8 at HNGAI, January-May 2021. Method: This study uses a quantitative approach, applied type, with an observational, cross-sectional, and non-experimental design. The sample consisted of 175 medical records. The instrument used was a data collection sheet where all the information from the medical records was recorded. The results were clear, aligned with the objectives, and supported by the statistical test used in this research, which is the Chi-square test with a significance level of 5%. Results: It was observed that the highest percentage of deceased patients admitted to the Intensive Care Unit were identified as adults (9.10%) and male (13.70%), with a duration of illness greater than 7 days (12.69%), moderate oxygen saturation levels (9.10%) and PaO2/FiO2 (9.10%), a severe percentage of pulmonary parenchyma involvement (6.90%), altered levels of C-reactive protein (16.00%) and D-dimer (13.10%), normal ferritin levels (11.40%), and a history of comorbidities, including obesity (3.40%), hypertension (1.70%), diabetes mellitus (1.70%), asthma (0.60%), and in some cases, up to two comorbidities (0.60%). Hypothesis testing determined that there is a statistically significant relationship between age and mortality (p: 0.01), duration of illness and mortality (p: 0.008), and C-reactive protein levels and mortality (p: 0.016). Conclusion: It was concluded that there is a statistically significant relationship between age and mortality, duration of illness and mortality, and C-reactive protein levels and mortality in COVID-19 patients.

    Conocimiento del personal profesional químico farmacéutico sobre nutrición parenteral en el Hospital Nacional Dos de Mayo, Lima 2020

    The objective of this thesis was to determine the level of knowledge of the professional pharmaceutical chemistry staff about parenteral nutrition at the Dos de Mayo National Hospital in Lima, Peru, in 2020. Methodology: The study used a non-experimental, descriptive, quantitative, and cross-sectional methodological design. A 19-question questionnaire was applied to 55 professional pharmaceutical chemists, who constituted 100% of the population at the hospital in 2020. Results: The results showed that the level of knowledge about the composition of parenteral nutrition mixtures and about the complications associated with the use of parenteral nutrition was regular at 41.8% and 36.4%, respectively. Conclusion: According to the results, the level of knowledge of the professional pharmaceutical chemistry staff about parenteral nutrition is regular at the Dos de Mayo National Hospital in 2020. However, it should be noted that the percentages for good and excellent knowledge levels surpass the regular level in both cases. This finding is consistent with the research's bibliographic references.

    Consumo de anticuerpos monoclonales en la unidad de mezclas oncológicas del Hospital Santa Rosa. Lima 2022

    Objective: To evaluate the consumption of monoclonal antibodies in the oncology mixing unit of Hospital Santa Rosa Lima, 2022. Method: This study has a quantitative approach, basic type, observational, cross-sectional, and retrospective design. It was applied to a sample of 245 treatment regimens. The instrument used was data collection sheets where all the information obtained from the chemotherapy regimens was recorded. The results were clear, aligned with the objectives, and supported by the bibliography. Results: The consumption of trastuzumab 440 mg was 97 vials, rituximab 100 mg was 43 vials, rituximab 500 mg was 20 vials, cetuximab 100 mg was 27 vials, and pertuzumab 420 mg was 4 vials dispensed. Cost results showed that trastuzumab 440 mg had the highest value at S/ 509,529.36, while the 6-month cost of rituximab 100 mg was S/ 197,748.40, rituximab 500 mg was S/ 48,607.60, cetuximab 100 mg was S/ 27,216.00, and pertuzumab 420 mg was S/ 29,732.00. Returns showed the highest index for trastuzumab 440 mg with 17 units returned during the 6-month study period, followed by rituximab 500 mg with 3 units. Savings results indicated that trastuzumab 440 mg achieved savings of S/ 89,298.96, and rituximab 500 mg achieved savings of S/ 7,291.14. Conclusion: The total consumption of vials from January to June 2022 was 191, with a total cost of S/. 812,833.36 soles, generating total returns of 20 vials, equivalent to savings of S/ 96,590.10 during this period.

    Controles rutinarios de nutriciones parenterales elaborados en la Unidad de Mezclas de Nutrición Parenteral del Hospital María Auxiliadora de Lima, octubre–noviembre 2021

    Objective: To identify errors in the preparation of parenteral nutrition through routine controls. Methodology: This was a prospective, descriptive study conducted during October and November 2022. The parenteral nutrition solutions were weighed, and their pH was measured. The theoretical weight was calculated based on the density and volume of each medication, with the acceptable percentage error for gravimetric accuracy being ±5% for preparations (≥300 ml) and ±3% for preparations (<300 ml). The medication control was conducted by comparing production sheets with medication consumption sheets. Results: A total of 384 parenteral nutrition solutions were analyzed, and 15 (3.9%) had gravimetric errors outside the acceptable range. Among the 221 preparations <300 ml, 11 (5.0%) were incorrect; and among the 163 preparations ≥300 ml, 4 (2.5%) were incorrect. All parenteral nutrition solutions had a pH within the acceptable range. In the medication control, 15 errors (1.3%) were detected: adding more volume of a medication in 7 cases (46.7%), adding less volume in 5 cases (33.3%), and using the wrong product in 3 cases (20%). Conclusions: Preparation errors were identified through routine controls, preventing them from being administered to patients. The most frequent gravimetric error occurred in preparations with a volume <300 ml, and all parenteral nutrition solutions had an appropriate pH.

    Objetivo: El cuidado humano es una necesidad diaria urgente en la práctica profesional y esto se refleja en las políticas y sistemas de las autoridades de salud que son las encargadas de velar por los derechos de los pacientes y velar por la calidad de los servicios prestados a los pacientes. Por otro lado, la satisfacción en la familia corresponde a los diferentes sentimientos y emociones que evoca el sujeto, por lo que todos los miembros del grupo pueden tener satisfacciones completamente diferentes; En otras palabras, la satisfacción familiar es una evaluación del estado mental generada por la familia en función del número de experiencias positivas y negativas experimentadas por el empleado responsable. El objetivo principal del estudio fue establecer la relación entre el humanitarismo y la satisfacción de familiares de pacientes moribundos en hospitales públicos - Chancay 2022.

    Disponibilidad de medicamentos y gestión en el suministro del Hospital Nacional Hipólito Unanue de enero–octubre 2020

    The arrival of the pandemic in our country has revealed that the health system presents numerous weaknesses; despite this, there was progress in the coverage of the Comprehensive Health System in line with the goal of universal health insurance. However, the management of the supply and availability of medicines continues to be a challenge for the national health system to overcome. The methodology had a quantitative approach of a basic type with a non-experimental, cross-sectional, descriptive, correlational, and retrospective research design. The population and sample consisted of 36 professional pharmaceutical chemists from a national hospital in Metropolitan Lima. The data collection technique was observation, and the instrument was the checklist. The results obtained for the variable availability of medicines showed that 22 professionals reported a high level, 11 a medium level, and 3 a low level; regarding the variable supply management, 61.1% indicated a high level of supply management, 30.6% a medium level, and 8.3% a low level. It is concluded that the availability of medicines is high due to the high level of supply management in a public hospital of Metropolitan Lima.

    Estudio de utilización de antibacterianos de reserva en las unidades de cuidados críticos e intermedios del Hospital Nacional Dos de Mayo durante 2012–2013

    The World Health Organization (WHO) defines drug utilization as “the marketing, distribution, prescription, and use of drugs in a society, with particular attention to the resulting medical, social, and economic consequences.” Therefore, drug utilization studies are essential to assess the benefit/risk ratio of active pharmaceutical ingredients, detect deviations and distortions, and consequently implement appropriate corrective measures. The objective of this study was to determine the consumption and expenditure of reserve antibacterials in the critical and intermediate care units of the Hospital Nacional Dos de Mayo during 2012–2013. Using the methodology recommended by the WHO for drug utilization studies in hospitals, data were presented in defined daily doses (DDD) per 100 bed-days (DDD/100 bed-days). Consumption data were obtained from the pharmacy management program, while the number of days, number of beds, and bed occupancy index were obtained from the Department of Statistics. During 2012–2013, it was determined that the carbapenem group ranked first in consumption in DDD/100 bed-days. Among these, meropenem was the most used in 2012, while imipenem + cilastatin was most used in 2013. Regarding expenditure, colistin accounted for 42% of the total cost during the study period. In conclusion, consumption and expenditure of reserve antibacterials quadrupled in 2013 compared to 2012; furthermore, no significant correlation was found between consumption and the bed occupancy index.

    Eventos adversos a dispositivos médicos en el Hospital San Juan de Lurigancho, 2018 a 2022. Lima 2023

    This research was conducted at the San Juan de Lurigancho Hospital in Lima, with the general objective of identifying adverse events related to medical devices reported at the hospital. The study was descriptive in type, with a non-experimental, cross-sectional design, using a deductive method and a quantitative approach. The population and sample consisted of 124 reports of suspected adverse incidents related to medical devices between 2018 and 2022. The data collection technique was documentary analysis, using a data collection form specifically designed for this research. Results: No severe adverse events were reported; 96.8% of the adverse events notified were mild, and 0.8% were moderate. Furthermore, 90.3% of adverse events occurred during the use of the medical device, and in 44.4% of cases, the probable cause was poor device quality. The main consequence of adverse events was device loss, which occurred in 72.6% of cases. Conclusion: At the San Juan de Lurigancho Hospital, between 2018 and 2022, most adverse events related to medical devices occurred in patients during device use, had mild consequences, and were likely attributable to poor device quality.

    Factores asociados a la adherencia al tratamiento en pacientes adultos con dislipidemia del servicio de consulta externa del Hospital Aurelio Díaz Ufano y Peral. Lima 2022

    The purpose of this research was to analyze the factors associated with treatment adherence in adult patients with dyslipidemia in the outpatient pharmacy service of Hospital Aurelio Díaz Ufano y Peral. The methodology used for this research was deductive with a quantitative approach and applied type. The population consisted of 135 individuals, with a representative sample of 101, and the technique used was a survey. A validated questionnaire was developed as the instrument, considering the following dimensions: socioeconomic factors, treatment-related factors, patient-related factors, service and healthcare personnel-related factors, and disease-related factors. The results showed that the most prevalent age group among adults with dyslipidemia in the outpatient service was those over 50 years old (29.7%), while the least frequent were those aged 18 to 30 years (21.8%). The most prevalent gender among adults with dyslipidemia was female (67.2%), while the least frequent was male (32.8%). Two factors, primarily (patient-related factors and disease-related factors), were associated with treatment adherence in adult patients with dyslipidemia. Conversely, two factors (treatment-related factors and service and healthcare personnel-related factors) were not associated with treatment adherence in these patients. Finally, it was concluded that female gender and age over 50 years are the most prevalent characteristics among adults with dyslipidemia. Moreover, the two main factors (patient-related and disease-related) were associated with treatment adherence in adult patients with dyslipidemia in the outpatient service of Hospital Aurelio Díaz Ufano y Peral.

    Factores asociados a los problemas relacionados con medicamentos (PRM) en pacientes hospitalizados en el servicio de medicina general del Hospital Santa Rosa en el año 2021

    Drug-Related Problems (DRPs) constitute a significant issue in the adherence to a patient's pharmacotherapeutic treatment. This research provides tools to identify and prevent the factors that contribute to their occurrence. The objective was to identify the factors associated with DRPs, using a non-experimental, case-control, prospective, and cross-sectional study. Results: A relationship was found between risk factors and the presence of DRPs in hospitalized patients in the General Medicine service of Santa Rosa Hospital. This was demonstrated in 10 variables: inappropriate drug selection, omission of necessary medication, incorrect administration frequency, incorrect administration route, age over 60 years, polypharmacy, renal insufficiency, hepatic insufficiency, narrow therapeutic margin drugs, and high-risk drugs. Statistical data processing was performed using Excel and SPSS, identifying the factors associated with DRPs. The main conclusion was that three factors were associated with DRPs: prescription factors, drug factors, and patient factors.

    Factores asociados y la adherencia terapéutica en pacientes hipertensos del Centro de Salud Túpac Amaru de Villa, Chorrillos, Lima Perú, 2022

    The general objective was: To determine the relationship between associated factors and therapeutic adherence in hypertensive patients at the Túpac Amaru de Villa Health Center, Chorrillos, Lima, Peru, 2022. Methodology: Hypothetical-deductive method, quantitative approach, basic type, non-experimental design, cross-sectional, descriptive correlational level, population: 296, with a non-probabilistic sample of 97 hypertensive patients with exclusion and inclusion criteria; ethical aspects were addressed. Results: 46.4% never presented associated factors, and 63.9% did not demonstrate therapeutic adherence. The Chi-Square correlation = 63.711 and p = 0.000 also indicate, according to the Cramer's V test = 0.810 (p = 0.000), statistically significant, showing a strong correlation between the variables. Conclusion: It has been determined that the factors associated with therapeutic adherence in hypertensive patients at the Túpac Amaru de Villa Health Center, Chorrillos, Lima, Peru, 2022, indicate, according to the correlation test ***p=0.000<0.050, with gl = 2 and Chi-Square = 63.711, a positive, very strong, and significant correlation between the variables.

    Gestión administrativa y desempeño laboral en los enfermeros del servicio de emergencia de un hospital público de Lima 2025

    The study titled "Administrative Management and Work Performance of Nurses in the Emergency Service of a Public Hospital in Lima 2025" aims to analyze the relationship between administrative management and the work performance of nursing staff in a high demand hospital setting. To achieve this, a non-experimental, cross-sectional, and correlational research design with a quantitative approach was used. The study was conducted with a census sample of 91 emergency service nurses, who were given two validated questionnaires: one to assess administrative management and another to measure work performance. The findings revealed a significant relationship between both factors, highlighting that planning and organization are key aspects in optimizing nursing work. Additionally, it was found that an efficient administrative environment positively impacts motivation, leadership, and responsibility among nursing staff. In conclusion, the study emphasizes the importance of strengthening administrative management in public hospitals, as this contributes to improving nursing performance and, consequently, the quality of patient care.

    Gestión de suministro en productos farmacéuticos, dispositivos e insumos médicos y satisfacción de pacientes atendidos en la farmacia del Hospital Nacional Hipólito Unanue–Lima, 2022

    The present research aimed to determine the relationship between supply management of pharmaceutical products, devices, and medical supplies, and patient satisfaction in the pharmacy of the National Hospital Hipólito Unanue – Lima, 2022. Methodology: A basic, non-experimental, correlational-level, hypothetical-deductive study with a quantitative approach was conducted. A total sample of 365 clients was taken from a population of 6,728. The survey technique was used, applying a 20-question questionnaire: 15 questions to evaluate supply management and 5 questions to assess patient satisfaction. SPSS 25 software and the Spearman correlation test were used for statistical analysis. The instrument was validated by three experts and obtained a Cronbach's alpha of α=0.887, which is interpreted as excellent reliability. Results: Based on the instruments applied and the variables studied, the inferential analysis showed a significance level of p=0.01 and a Spearman correlation coefficient of Rho=0.580 (moderate positive). For the dimensions studied: planning showed Rho=0.465 with a significance of p=0.01, supply showed Rho=0.404 with a significance of p=0.01, and storage showed Rho=0.461 with a significance of p=0.01. Conclusion: There is a moderate positive relationship between the supply management of pharmaceutical products, devices, and medical supplies and patient satisfaction in the pharmacy of the National Hospital Hipólito Unanue, Lima 2022.

    Implementación de una unidad de mezclas de nutrición parenteral y beneficios en pacientes adultos críticos de un hospital nivel III de Huancayo de enero 2018 a enero 2021

    Objective: To determine the benefits of implementing the parenteral nutrition support unit in critically ill patients at a Level III Hospital in the city of Huancayo from January 2018 to January 2021. Methodology: This is a descriptive, comparative study. Data collection was based on medical records extracted from the pharmacy department's information system and the statistics department. The Student's t-test and chi-square test were used. Results: Before implementing the nutritional support unit, 29.8% of patients remained hospitalized for more than 22 days, which decreased to 9.5% after implementation. The percentage of patients with a BMI lower than 18.5% upon discharge dropped from 56.7% before the implementation to 17.9% afterward. The albumin level at discharge increased from 2.4 mg/dl to 3 mg/dl before and after the implementation of the nutritional support unit, a difference that was statistically significant (p < 0.05). Additionally, post-implementation, wound infections decreased from 19.3% to 7.4%, pressure ulcers from 11.7% to 6.3%, and mortality from 17.5% to 7.4%. Conclusions: It was determined that implementing the nutritional support unit is beneficial for critically ill patients in the Intensive Care Unit.

    Inteligencia espiritual y bienestar psicológico en enfermeras de un hospital público, 2024

    La inteligencia espiritual se ha visto cada vez más como la base de la existencia humana y la curación. El cuidado holístico que es un pilar en enfermería, adopta la filosofía de tratar el bienestar físico, mental y espiritual de una persona. Estudios muestran que, los individuos con alto y bajo inteligencia espiritual presentan resultados sustancialmente diferentes, por lo que el bienestar psicoespiritual se ha convertido en un área de interés. El objetivo del presente trabajo es determinar la relación entre la inteligencia espiritual y el bienestar psicológico en enfermeras de un hospital público, 2024. El trabajo se desarrollará mediante un método hipotético deductivo, diseño no experimental, correlacional, nivel descriptivo, enfoque cuantitativo y corte transversal. La población y muestra del estudio estará conformada por 90 profesionales enfermeros de un hospital público. La técnica a utilizar será cuestionario y el instrumento para la recolección de datos (SISRI 24) para medir inteligencia espiritual y escala de bienestar psicológico de Ryff para el segundo constructo. Para los resultados, los datos obtenidos de las muestras serán analizados y presentados en Excel utilizando el programa SPSS 27.0, a fin de sintetizar los resultados y proporcionar tablas y gráficos para interpretarlos y determinar las conclusiones finales.

    Interacciones medicamentosas en pacientes atendidos en el servicio de medicina interna del Hospital Daniel Alcides Carrion. Callao 2022

    The method of this study was observational, cross-sectional, and non-experimental. Information was collected through medical prescriptions. The study population consisted of 500 medical prescriptions, with a sample of 220 prescriptions. The instrument used was a data collection sheet, which included four dimensions: Type of Interactions, Pharmacotherapeutic Group, Severity Grade, and Number of Interactions. The Micromedex database was utilized. The results showed that in terms of medication interactions, the most influential in the evaluated prescriptions were Pharmacodynamic interactions, accounting for a total of 150 (68%). Regarding the Pharmacotherapeutic Group, antibiotics were the most commonly observed group, with 47 (21.0%). For the Severity Grade, most cases were moderate, with 141 (64%). Additionally, for the Number of Interactions, the most influential was prescriptions with 1 interaction, at 79 (36.0%). It was concluded that 76% of the 220 analyzed medical prescriptions presented medication interactions.

    Interacciones medicamentosas reales y potenciales de los psicofármacos en los pacientes del servicio de medicina COVID-19 del Hospital de Emergencia de Villa el Salvador Lima 2021

    The objective of this study is to identify "the actual and potential drug interactions of psychotropic medications in patients in the COVID-19 Medicine Service at the Villa El Salvador Emergency Hospital during the months of February to July 2021." The study is deductive, retro-prospective, quantitative, applied, and observational with a cross-sectional design. A data collection form was used for 87 pharmacotherapeutic follow-ups in which psychotropic medications were prescribed for anxiety, depression, and insomnia. The results showed that the most significant indicator accounted for the highest frequency with 307 (89%) interactions. Regarding the pharmacodynamic indicator, it presented the highest frequency with 183 (53%) interactions. The "justified" indicator had the highest frequency with 252 (73%) interactions. The potential interaction indicator showed a higher frequency with 318 (92.2%) interactions. The rapid interaction indicator had the highest prevalence with 19 (5.5%) interactions. As for the probable interaction indicator, it presented the highest frequency with 25 (7.25%) actual interactions. Regarding gender, males exhibited 172 (49.9%) potential interactions, while females showed a higher incidence of actual interactions with 15 (4.3%) interactions. The average age was 48.58 years; for potential interactions, it was 48.83 years, and for actual interactions, it was 45.67 years. Sertraline accounted for 184 (53.2%) potential interactions, while mirtazapine showed the highest frequency of actual drug interactions with 12 (3.5%) interactions. It was concluded that the increase in psychotropic prescriptions is related to a higher likelihood of drug interactions.

    Manejo de la cadena de frío en productos farmacéuticos por profesionales de la salud del Hospital María Auxiliadora. San Juan de Miraflores 2022

    Objective: To evaluate the management of the cold chain for pharmaceutical products by healthcare professionals at María Auxiliadora Hospital, San Juan de Miraflores, 2022. This study used a descriptive methodology with a quantitative approach, observational, cross-sectional, and non-experimental design. Data were obtained through a questionnaire and processed using the statistical software SPSS STATIST version 26. Results: The participation rate was 100%, revealing that 26.6% of healthcare professionals have a high level of knowledge about cold chain management for pharmaceutical products, 51.9% have a medium level, and 21.5% have a low level of knowledge about cold chain management for pharmaceutical products. Conclusion: Based on the applied instrument, it was determined that healthcare professionals have a medium level of knowledge and generally adhere to processes and standards for cold chain management. However, there are healthcare professionals whose improper handling of the cold chain during storage, distribution, and transportation may cause harm and health losses.
