    Benefits of oropharyngeal colostrum administration in very low birth weight preterm infants

    Oropharyngeal administration of colostrum consists of promoting the immunocompetence of cytokines contained in colostrum in the oropharynx, in the lymphoid system with which it has a relationship with the intestine, in order to provide a barrier against bacterial colonization. The present secondary research titled as a critical review: Oropharyngeal Colostrum Positively Modulates the Inflammatory Response in Preterm Neonates aimed to evaluate the effects of oropharyngeal breast milk administration on inflammatory signaling in extremely premature babies. The clinical question was: Is there a positive relationship between the administration of oropharyngeal colostrum in very low weight premature newborns on inflammatory markers? The CASPE critical reading tool methodology was used for the Clinical Trial, which has a level of evidence I and a strong Grade of Recommendation, according to the researcher's expertise. The critical comment allowed us to conclude that administering colostrum during the first days would have a therapeutic effect at an anti-inflammatory level, so starting at an opportune time could contribute positively to the tolerance of oral feeding.

    Benefits of the Kangaroo Mother Method in preterm newborns of 34 to 36 weeks in the intermediate care unit of a specialized institute, 2020

    The present research project, titled “Knowledge About Newborn Care in Adolescent Mothers, Specialized Institute of Lima, 2020”, aims to determine the knowledge about newborn care in adolescent mothers. The study evaluates knowledge across three dimensions: physical care, warning signs, and early stimulation of newborns. The research approach is quantitative, with data quantifiable for statistical analysis. The study is descriptive and cross-sectional, providing information as it is obtained. The sample consists of 80 adolescent mothers who met the inclusion and exclusion criteria. The technique used will be the survey, and the data collection instrument is a structured questionnaire. The conclusion is based on evidence from national and international studies showing that most adolescent mothers have an average knowledge level about newborn care.

    Complicaciones del soporte nutricional enteral en recién nacidos prematuros de la unidad de neonatología hospital II Lima Norte Callao Luis Negreiros Vega, Callao 2022

    La presente investigación titulada “Complicaciones del soporte nutricional enteral en recién nacidos prematuros de la Unidad de Neonatología Hospital II Lima Norte Callao Luis Negreiros Vega, Callao 2022”. Objetivo: Determinar las complicaciones del soporte nutricional enteral en recién nacidos prematuros. Investigación básica, transversal, hipotético deductivo, cuantitativo. La muestra: 169 y el instrumento: lista de recolección de datos. Resultados: El 50,3% fueron masculino y el 49,7% femenino. El 58% pesaban de 2500 g a más, el 39,1% de Bajo peso (1500 - 2499 g) El 85,2% tenían de 34 a 36 semanas; el 14,2% tenían 32 a 34 semanas. El 82,8% recibió nutrición mixta, el 13,6% recibieron leche materna y el 3,6% recibieron fórmula infantil. El 100% no presentaron complicaciones mecánicas. El 99,4% no presentaron complicaciones metabólicas. El 100% no presentaron complicaciones infecciosas. El 85,8% no presentaron complicaciones gastrointestinales, 11,2% presentaron diarrea, 2,4% distensión abdominal y el 0,6% nauseas. Conclusiones: El mayor porcentaje fue masculino, pesaban de 2500 g a más; tenían de 34 a 36 semanas y recibieron nutrición mixta. El 100% no tuvieron complicaciones mecánicas ni infecciosas. El 99,4% no tuvieron complicaciones metabólicas. El 85,8% no tuvieron complicaciones gastrointestinales. La mayoría, no presentaron complicaciones metabólicas ni gastrointestinales.

    Critical review: protective effect of breast milk in preterm infants”

    The benefits of breast milk on the health of premature newborns is widely known and the administration of human milk to infants as an ideal food over any other is of great clinical and social relevance. The protective effect against necrotizing enterocolitis (NEC) has been very well studied, but it was necessary to compare different doses of breast milk as a protective effect. The present secondary research titled as a critical review: Protective effect of breast milk in premature newborns, aimed to evaluate the protective effect of breast milk in premature newborns. The clinical question was: What is the protective effect of breast milk in premature newborns? The Evidence-Based Nutrition (NuBE) methodology was used. The information search was carried out in PUBMED, SCIENCE DIRECT, COCHRANE, finding 128 articles, 25 being selected that have been evaluated by the CASPE critical reading tool, finally selecting the meta-analysis titled Protective effects of different doses of human milk on neonatal necrotizing enterocolitis . Which has a level of evidence A II and a strong Degree of Recommendation, according to the expertise of the researcher. The critical comment allowed us to conclude that breast milk exerts a protective effect against necrotizing enterocolitis at any dose, even more so if it is exclusive breast milk.

    Effectiveness of "Plastic Wrap" in preventing hypothermia in preterm newborns

    Objective: Systematize the evidence on the effectiveness of Plastic Wrap in preventing hypothermia in premature newborns. Material and Methods: Systematic review of 10 selected articles, using the Pubmed, Elsevier, Science Direct, Redalyc, Cochrane, Researchgate database. It is a quantitative study with a systematic experimental and cohort design. Results: The effectiveness of Plastic Wrap in preventing hypothermia in premature newborns. We found that 100% (n = 10/10) of the articles evidence the effectiveness of Plastic Wrap in preventing hypothermia in premature newborns. The effectiveness in preventing hypothermia in premature newborns together with other devices represents 50% (n=5/10), making skin-to-skin contact and using heat-transmitting mattresses that keep premature newborns warmer, which leads to stable temperatures upon admission to neonatal units and less hypothermia. Conclusion: Plastic Wrap demonstrates effectiveness in preventing hypothermia in premature newborns.

    Effectiveness of developmentally focused care for improving the hospital stay of preterm neonates in neonatal units

    A systematic, observational, and retrospective review of a quantitative type, subject to analytical classification using the GRADE evaluation system to identify the level of evidence, found in the following databases: PubMed, Cochrane, Scielo, Google Scholar, Dialnet, Elsevier. Of the 10 articles systematically reviewed, 60% (n=6/10) are systematic reviews, 20% (n=2/10) are randomized controlled trials, 10% (n=1/10) are cross-sectional studies, and 10% (n=1/10) is a qualitative-quantitative observational study. According to the results from the systematic review conducted in this research, 60% of the evidence comes from Spain, 10% from Costa Rica, 10% from Mexico, 10% from the United States, and 10% from Colombia. Of the evidence found, 70% (n=7/10) indicates that developmentally focused care is effective in improving the hospital stay of preterm newborns in neonatal units, while 30% (n=3/10) suggests that developmentally focused care is not effective in improving the hospital stay of preterm newborns, but has qualitative value in terms of humanization, family, and preterm newborn care.

    Effectiveness of early use of continuous positive airway pressure in preventing bronchopulmonary dysplasia in premature newborns

    The present research work is based on the review of 10 articles, 80% (8/10) articles show that the early use of Continuous Positive Airway Pressure (CPAP) in premature babies without apnea is a valid strategy. , to the extent that in more than 80% of cases it provides sufficient treatment and of shorter duration than that required if mechanical ventilation were immediately applied, which also reduces the risk of lung injury associated with this assistance respiratory. This means that in a very good part of these patients the problem is resolved with less exposure to oxygen, avoiding the consequences of mechanical ventilation, consequently reducing the incidence of central nervous system hemorrhage, sepsis, bronchopulmonary dysplasia, without increasing the risk of pneumothorax. Concluding that the early use of CPAP is a safe, low-cost method and an effective therapy in premature neonates with respiratory difficulty. It reduces in-hospital morbidity and mortality, reduces the incidence of complications, such as Bronchopulmonary Dysplasia, and is an alternative to routine intubation and the need for mechanical ventilation.

    Effectiveness of high-frequency oscillatory ventilation versus continuous positive airway pressure in reducing complications in premature neonates with respiratory distress syndrome

    Effectiveness of low oxygen levels in preventing retinopathy in premature infants

    In the review described, we can detail that 10 journals were taken to describe the goal of the development of this work, where 7 of these journals concluded that the use of low level of oxygen (85-94%) in the premature neonate was very beneficial for the prevention of ROP, suggesting that it be increased progressively and according to the age of the newborn; 2 of the 10 studies report that no benefit was found and rather a higher rate of morbidities associated with high mortality was found. Finally, only one study indicates that it did not find any difference between the use of low or high o2 to be associated with ROP. Therefore, it could be stated, despite the lack of evidence in our reality, that using O2 at a low level and increasing it gradually according to the weight gain of the premature neonate; and adapt it to our protocols for the management of premature infants by specialized nursing in the NICU at the local level, being certain that this management is very beneficial to considerably reduce the visual morbidity that is ROP in premature infants.

    Effectiveness of music therapy in promoting weight gain in premature newborns

    Of the 10 articles reviewed, it is shown that the use of music therapy to promote weight gain in preterm neonates is effective. It is concluded that, of the 10 scientific articles reviewed in the neonatal unit, care should be comprehensive. Advances in care and management with music therapy to promote weight gain in preterm neonates are effective, as they improve pain and stress reduction, decrease energy expenditure, enhance feeding, regulate physiological levels, reduce heart rate and respiratory rate, improve resting energy expenditure, and increase oxygen saturation.

    Effectiveness of the kangaroo mother method compared to the conventional method for the prevention of heat loss in preterm infants under 2000 grams

    Through the Kangaroo Mother Method, care and attention are provided to low birth weight babies. It consists of two fundamental pillars: breastfeeding and skin-to-skin contact. It is an effective, easy-to-implement, cost-effective technique that meets the newborn's needs and serves as an optimal alternative to incubator care. OBJECTIVE: To systematize the evidence found on the effectiveness of the Kangaroo Mother Method compared to the conventional method for preventing heat loss in neonates weighing less than 2000 grams. MATERIAL AND METHOD: Observational and retrospective systematic review. The search was limited to articles that include studies on the benefits of the Kangaroo Mother Method to ensure neonatal survival. Selected studies underwent critical analysis. The quality of the evidence and strength of the recommendations were assessed using the GRADE system. RESULTS: 100% of the reviewed articles are conclusive, supported by 3 systematic reviews, 3 randomized clinical trials, 1 quasi-experimental study, and 3 cohort studies; all suggest the use of the Kangaroo Mother Method to prevent heat loss in neonates weighing less than 2000 grams. CONCLUSIONS: The Kangaroo Mother Method is highly effective for newborns weighing less than 2000 grams as an alternative to conventional neonatal care to prevent heat loss.

    Effectiveness of the use of probiotics for the prevention of necrotizing enterocolitis and mortality in preterm infants

    Objective: To systematically analyze the evidence on the efficacy of probiotics for the prevention of necrotizing enterocolitis and mortality in preterm newborns. Material and method: Eleven scientific articles on the efficacy of probiotics for the prevention of necrotizing enterocolitis and mortality in preterm newborns were found in the databases Cochrane, Scielo, Lilacs, and Medline/Pubmed. A systematic review was applied, analyzing the studies using the GRADE scale to verify their strength and quality of evidence. Of the 11 articles reviewed, 36% (4/11) correspond to the methodological design of systematic reviews, 55% (6/11) are meta-analyses, and 9% (1/11) is a systematic review and meta-analysis. Results: 82% (9/11) demonstrate the efficacy of probiotics for the prevention of necrotizing enterocolitis and mortality in preterm newborns, and 18% (2/11) show that probiotics prevent only necrotizing enterocolitis. Conclusions: From the 11 articles reviewed, the efficacy of probiotics for the prevention of necrotizing enterocolitis and mortality in preterm newborns is evidenced.

Results: Of the 10 articles reviewed, 90% (9/10) show that the use of therapeutic massage in preterm newborns is effective in increasing weight, while 10% of the studies conclude that therapeutic massage does not promote weight gain in this population. Conclusion: Based on the reviewed evidence, we can conclude that therapeutic massage is effective in increasing the weight of preterm newborns.

    Effectiveness of thermal mattresses in preventing hypothermia in premature or low-birth-weight newborns

    OBJECTIVE: To systematically analyze the evidence on the effectiveness of the thermal mattress in preventing hypothermia in premature or low birth weight newborns. MATERIAL AND METHOD: This study uses a systematic review of 10 articles about the effectiveness of the thermal mattress in preventing hypothermia in premature or low birth weight newborns, which were found in the following databases: Pubmed , Sciencedirect, Epistemonikos, Cochrane Plus, Medline, Dialnet, Scielo. All articles used in this review were analyzed according to the Grade scale to determine their strength and quality of evidence. RESULTS: 100% of reviewed articles are conclusive. However, some authors suggest the use of a thermal mattress used autonomously and others that it be combined with some other intervention, but always monitoring the temperature to prevent iatrogenic hyperthermia. CONCLUSIONS: 100% (10/10) of the scientific articles in this study show that the use of thermal mattresses in premature and low birth weight newborns; It is effective for the prevention of hypothermia. The use of the thermal mattress simultaneously with the polyethylene bag provides warmth to the premature or low birth weight newborn, preventing hypothermia, but continuous monitoring must be carried out to avoid iatrogenic hyperthermia.

    Efficacy of early stimulation to promote neurodevelopment in the premature newborn.

    Objective: To systematize the evidence found on the effectiveness of early stimulation in promoting neurodevelopment in preterm newborns. Materials and methods: This work was developed within the framework of a systematic review, a design based on the GRADE evaluation system, which allows evaluating the quality of the selected scientific articles from databases such as PubMed, ScientDirect, EBSCO, Elsevier, and Cochrane. Results: The reviewed articles showed that 60% were systematic reviews, 10% were randomized controlled trials, 10% were clinical trials, 10% were systematic reviews and meta-analyses, and 10% were meta-analyses. Additionally, 80% of the articles found evidence supporting the effectiveness of early stimulation to promote neurodevelopment in preterm newborns, while 20% did not show evidence. Conclusion: Of the 10 articles systematically reviewed, 8/10 found that early stimulation through therapeutic massage, aquatic exercises, and passive physical activities is effective in promoting neurodevelopment in preterm newborns, improving factors such as motor, cognitive, behavioral, neuromuscular, and functional development, especially within the first three months of intervention. In 2/10, no effectiveness was found.

    Environmental factors and stress level in premature newborns in the neonatal intensive care unit of a Loreto Hospital - 2024

    Introduction: The study aims to provide an overview of the relationship between environmental factors and stress levels in preterm infants admitted to the Neonatal Intensive Care Unit (NICU) of a hospital in Loreto. In a NICU, the newborn faces a highly stimulating environment full of environmental aggressions, such as noise and intense light, which may interfere with their development and adaptability. Objective: The present study seeks to determine the relationship between environmental factors and stress level in premature newborns in the Neonatal Intensive Care Unit of a Loreto Hospital - 2024. Methodology: Similarly, the study will employ a quantitative approach using a hypothetico-deductive method. It will be applied research, focused on a correlational level, and will use a non-experimental design with a population of 80 premature neonates, whose number will also be the same amount in the same conditions; it will work with two validated and reliable observation cards. For data collection, observation cards will be applied, which will take approximately 25 minutes per patient. The resulting data will be analyzed using the SPSS 25.0 statistical program, which will generate tables and graphs to interpret the behavior of the variables and their relationships. Spearman's Rho test will be used to test the hypotheses and draw conclusions from the results.

    Factores asociados a complicaciones del uso de nutrición parenteral en recién nacidos prematuros del Hospital El Carmen Huancayo 2022

    El Objetivo: Identificar los factores asociados a las complicaciones del uso de nutrición parenteral en pacientes prematuros del hospital el Carmen Huancayo 2022. La Metodología. Corresponde a un estudio con enfoque Cualitativo, deductivo, de tipo básico, diseño, no experimental, descriptivo y correlacional., la información se recolecto de las historias clínicas de los pacientes que cumplían con los criterios de inclusión y exclusión. Los Resultados demostraron que de los 38 recién nacidos prematuros, el 39,5% presentó complicaciones mecánicas, el 34,2% complicaciones metabólicas, y el 26.3% complicaciones infecciosas, entre los factores relaciones se identificó que el 52,6% se encontraba en edad gestacional entre 28 a 32 semanas, el 57,9% pertenecía al sexo masculino, el 47,4% con bajo peso al nacer, el 59% de pacientes que recibieron nutrición parenteral por más de 14 días presentaron más complicaciones. Conclusión: El tipo de complicación con mayor prevalencia en los pacientes recién nacidos neonatos fue la complicación mecánica con un 39,5%, siendo la complicación más frecuenta por obstrucción del catéter con un 46,7%, y los factores más frecuentes que produjeron más complicaciones en los prematuros fueron; el peso al nacer, el tiempo de administración (>14dias), la edad gestacional entre 28 a 32 semanas (muy prematuro), la vía de administración periférica.

    Knowledge of enteral nutrition of the premature newborn in the practice of nurses in the neonatal intensive care unit service of a Hospital in Lima, 2024

    The main objective of this research is to determine how the level of knowledge about enteral nutrition impacts the practice of nursing professionals in hospitalized neonates in the Neonatal Intensive Care Unit of a hospital in Lima. It is a study with a quantitative approach, using a hypothetical-deductive method, basic type, with a non-experimental, descriptive, and cross-sectional design. The sampling method to be used will be census-based due to the small population size. To measure knowledge, the questionnaire titled "Knowledge about premature enteral nutrition," created by Sea Elizabeth in La Paz, Bolivia, will be used. This questionnaire consists of 20 questions structured into two dimensions. To evaluate nursing practices in enteral nutrition, an observation guide created by the same author, Sea Elizabeth, will be applied. It consists of 27 items structured into three phases: before, during, and after enteral nutrition, developed on a dichotomous scale. The results will be collected using previously validated instruments, processed, coded, and subsequently entered into a database designed using Excel.

    Premature babies are usually admitted to the Neonatal Intensive Care Unit, due to their long hospital stay they require the use of long-term venous access devices. The peripherally inserted central catheter (PICC) is an advance in long-stay intravenous therapy and its use is currently increasing, it is more comfortable for the patient, fewer punctures and complications, its insertion is performed by a Nurse. Objective: To determine the Level of Knowledge of the Nursing professional in the management of the peripherally inserted central catheter (PICC), in the Neonatal Intensive Care Unit San Juan Bautista Hospital - Huaral 2021 Methods: The descriptive, quantitative and cross-sectional study. The population and sample will be made up of 12 Nurses specialized in Neonatal Intensive Care, who work in the NICU, in the third quarter of 2021 The instrument: the Likert-type questionnaire with 26 closed questions. data analysis: using tables and graphs, using the Microsoft Excel program.
