    Anemia y efectividad del tratamiento con suplementos de hierro en población pediátrica del Hospital Carlos Lanfranco la Hoz Puente Piedra - Lima-Peru, 2024

    La investigación tiene como Objetivo determinar la correlación entre la anemia y la efectividad del tratamiento con suplementos de hierro en niños menores de 5 años del Hospital Carlos Lanfranco La Hoz, Puente Piedra, Lima-Perú. Metodología. estudio retrospectivo de cohortes, de tipo descriptivo-analítico, utilizando datos de historias clínicas de 100 niños menores de 5 años diagnosticados con anemia y tratados con suplementos de hierro en el hospital durante los meses de enero a mayo de 2024, muestreo no probabilístico por conveniencia analizando el software SPSS versión 25, empleando la prueba estadística V de Cramer para determinar la correlación entre las variables. Resultados. muestran una fuerte correlación entre la severidad de la anemia y la efectividad del tratamiento. Mientras que en niños con anemia normal el 80% mostró una efectividad buena, este porcentaje disminuyó a 52.38% en anemia leve, 56.67% en anemia moderada y 50% en anemia severa. Además, la proporción de efectividad mala aumentó con la severidad de la anemia (20%, 21.43%, 26.67% y 38.89% para anemia normal, leve, moderada y severa, respectivamente). Conclusión. existe una correlación significativa entre la anemia y la efectividad del tratamiento con suplementos de hierro en población pediátrica del hospital Carlos Lanfranco La Hoz Puente Piedra - Lima-Perú, 2024.

    Clinical benefits of early removal of dressings on clean surgical wounds

    Objective: Systematize the evidence on the clinical benefits of early removal of dressings in clean surgical wounds. Materials and Methods: In the final selection, 10 studies were chosen, the systematic review of the 10 scientific articles on Clinical benefits of early removal of dressings in clean surgical wounds, were found in the following databases Lilacs, Pubmed, Medline and Cochrane Plus , Systematic Review 05, Randomized Clinical Trials 04 and Cross-sectional 01 were included. Results: The 10 reviewed articles, the 70% (n=07/10) of these show that early removal of dressings in clean surgical wounds has clinical and economic benefits, such as a shorter hospital stay and a significant reduction in costs; 30% (n=03/10) affirm that there is not enough scientific evidence to recommend early removal of dressings in clean surgical wounds. Conclusions: Of the 10 articles reviewed, 70% (n=07/10) of these (8,9,10,11,12,13,17) agree that there is evidence that early removal of dressings in surgical wounds clean has clinical and economic benefits, such as shorter hospital stay and a significant reduction in costs; 30% (n=03/10) of the authors of the studies (14,15,16) state that there is insufficient scientific evidence to recommend early removal of dressings in clean surgical wounds.

    Clinical benefits of late umbilical cord clamping in neonates

    Objective: Systematize the clinical benefits of late clamping of the umbilical cord in neonates. Materials and Methods: Systematic Review is made up of 10 scientific articles on the benefits related to late clamping of the umbilical cord in neonates, which were found in the Lilacs, Scielo, Pubmed, Medline and BVS databases. Results: In the review carried out, 90% of the studies reviewed agree that late clamping of the umbilical cord improves the hematological status and adaptation of the newborn without complications. 10% report that there are no significant differences between one practice or another, so the most physiological should be practiced. Conclusions: 100% of the articles reviewed demonstrate the benefits related to late clamping of the umbilical cord in neonates, improving hematological status, thus reducing the risk of neonatal anemia, specifying that early clamping is not associated with neonatal anemia.

    Clinical benefits of peritoneal dialysis in the treatment of patients with refractory heart failure

    This is a systematic review of 10 investigations at the national and international level, where it was found that 60% of the investigations found correspond to Spain, 20% Argentina, 10% the United States and 10% Colombia. The majority have been analyzed: 40% descriptive studies, 20% experimental studies and 10% systematic reviews. Results: 90% of the studies show that peritoneal dialysis is a good option in the management of refractory heart failure, and 60% show benefits in functional class, improvement in costs and effectiveness, decrease in mortality, decrease in hospital admissions, it also achieves a positive impact on the quality of life reported by the patient. Conclusions: peritoneal dialysis benefits the patient in functional class in 60% of patients with heart failure.

    Effectiveness of conventional hemorrhoidectomy versus hemorrhoidopexy in improving recovery in patients with hemorrhoids

    Objective: Systematize the evidence on the effectiveness of conventional hemorrhoidectomy versus hemorrhoidopexy in improving the recovery of patients with hemorrhoids. Materials and Methods: The type of research was quantitative, the study design was through systematic review, the population was 20 articles, and the sample used was 10 scientific articles published and indexed in scientific databases, the instrument was a search based of data: Scielo, PubMed, EBSCO and Epystemónikos. For the evaluation of the articles, the GRADE System was used, which evaluated the quality of evidence and the strength of recommendation. Results: Hemorrhoidopexy and Hemorrhoidectomy show equal postoperative satisfaction and did not have a significant difference in the assessment of postoperative pain and bleeding; hemorrhoidectomy is more effective, with less recurrence of the disease compared to hemorrhoidopexy; Patients undergoing hemorrhoidopexy showed shorter hospital duration compared to hemorrhoidectomy. Conclusions: 50% evidence the effectiveness of both surgical techniques, both conventional hemorrhoidectomy and hemorrhoidopexy. Likewise, 30% effectiveness corresponds to hemorrhoidopexy and 20% corresponds to conventional hemorrhoidectomy.

    Effectiveness of early administration of antibiotic therapy to decrease mortality in patients with severe sepsis in emergency.

    Introduction: According to the World Health Organization (WHO), approximately 31 million people suffer from an episode of sepsis each year. Of these, approximately 6 million people die from sepsis. In low- and middle-income countries, the burden of sepsis is higher and represents one of the main causes of maternal and neonatal death. (5) In Peru, there are no systematized national epidemiological data on morbidity and mortality associated with sepsis. In primary studies carried out in different hospitals in Peru, it was determined that the mortality rate associated with sepsis varies from 25.3% to 40%, increasing according to the risk factors associated with each population studied.(6)

    Effectiveness of high-flow nasal cannula in patients with bronchiolitis for improving clinical outcomes

    Objective: Systematize and analyze the evidence on the effectiveness of the use of high-flow nasal cannula in patients with bronchiolitis. Material and Methods: 10 scientific articles published in different databases were taken, the GRADE system was used to classify the quality of evidence and estimate the strength of recommendation, the studies come from: Australia, Norway, USA, Spain and Atlanta. Of the 10 articles, 20% (n= 2/10) are systematic reviews, 20% (n= 2/10) are randomized controlled studies and 60% (n= 6/10) are cohort studies. Results: With the High Flow Nasal Cannula (HFNC) oxygen saturation improves in the first 12 hours, the need for intubation in patients with bronchiolitis is reduced by 8.4%; There is a 12% rate of escalation of care in infants with HFNC due to failure; Those older than 12 months respond well, as do infants with bronchiolitis, to the use of HFNC; 13% of failure is due to metabolic failures; Of the total failures, 28% go to CPAP and 11% to be intubated. Conclusion: Of the 10 articles, 9/10 determine that the use of HFNC in patients with bronchiolitis is effective in improving the clinical picture, managing to reduce R.F., increase oxygen P/A, decrease HR and increase O2 saturation. . And 1/10 points out that it is not effective, since the study had a limited population, there is a lack of more studies that guarantee the effectiveness of the high-flow nasal cannula in emergencies.

    Effectiveness of intravenous treatment to reduce complications in post-operative patients for venous insufficiency

    Objective: Analyze and systematize the evidence on the effectiveness of intravenous treatment to reduce complications in post-operative patients for venous insufficiency. Materials and Methods: The type of research was quantitative and the study design is a systematic review, the population was a total of 20 scientific articles which only 10 scientific articles published and indexed in scientific databases were sampled, to The GRADE method was used to evaluate the articles, which evaluated the quality of evidence and the strength of recommendation. Results: Of 100% of the articles reviewed, 10% correspond to Holland, China, South Korea and Colombia respectively and 20% correspond to Brazil, Chile and Egypt. In relation to the type of study, it was 100% quantitative and the study design corresponds to 10% Cohort, 20% retrospective clinical trial and 70% prospective randomized clinical trial. Conclusions: It is concluded that 90% of the studies show that the effectiveness of intravenous treatment in reducing complications in post-operative patients with venous insufficiency improves short-term results.

    Effectiveness of non-pharmacological interventions to reduce pruritus in patients with chronic kidney disease

    To systematically analyze the available evidence from studies carried out on the effectiveness of non-pharmacological interventions to reduce pruritus in patients with chronic kidney disease. MATERIAL AND METHOD: An observational, methodical analysis, which were selected through the Grade evaluation to determine their strength and quality of evidence, found in: Epistemonikos, Lilacs, Pubmed. Of the 10 articles analyzed structurally, 80% (n=8/10) correspond to the methodological design of systematic review and randomized controlled trial and 20% (n=2/10) are case-control studies. The base of results found from these studies are from the countries of SINGAPORE (10%), China (10%), IRAN (40%), Taiwan (10%), Indonesia (10%), South Korea ( 10%) and Egypt (10%). RESULTS: 80% (8/10) evidence the effectiveness of non-pharmacological interventions to reduce pruritus in patients with chronic kidney disease. CONCLUSIONS: Non-pharmacological interventions are effective for the treatment of pruritus in patients with chronic kidney disease.

    Effectiveness of psychosocial nursing interventions for improving the treatment of patients with psychiatric disorders

    Systematize the evidence found on the effectiveness of psychosocial nursing interventions to improve the treatment of patients with psychiatric disorders. Material and Methods: Systematic review, based on critical reading and carried out under the GRADE evaluation system to evaluate the quality of the evidence of the articles selected from the following databases: Epistemonikos, PubMed, Sciencedirect, Elsevier, Cochrane and Scielo. Results: The reviewed articles showed that 70% are systematic reviews, 20% systematic reviews with meta-analysis, and 10% integrative reviews. On the other hand, it was found that in 90% there is effectiveness of psychosocial nursing interventions to improve the treatment of patients with psychiatric disorders, while 10% showed that there is no effectiveness. Conclusion: Of the 10 articles systematically reviewed in (9/10), it was found that psychosocial nursing interventions in patients with psychiatric disorder were effective; such as peer support, family or caregiver interaction, psychoeducational interventions, exercises, recreational activities, among others and only (1/10) showed that there is no effectiveness.

    Effectiveness of the Cyriax deep transverse friction technique in patients with lateral epicondylitis at the José Casimiro Ulloa Emergency Hospital, 2021

    Epicondylitis is the leading cause of lateral elbow pain, becoming a major health issue due to the pain it causes the individual and the subsequent work disability it leads to, as patients must alter their daily activities to avoid the pain. Most patients seek healthcare when the pathology has progressed, making recovery slow and potentially leading to recurrences. Therefore, since Cyriax’s deep transverse friction is a common technique in manual therapy, its effectiveness in cases of lateral epicondylitis needs to be researched.

    Effectiveness of the use of high-flow nasal cannula in improving the clinical picture in patients with chronic obstructive pulmonary disease (COPD)

    Systematize and analyze the evidence on the effectiveness of the use of high-flow nasal cannula in improving the clinical picture in patients with Chronic Obstructive Pulmonary Disease (COPD). Material and Method: It was developed through the systematic review of scientific articles based on the PICO methodology. These articles maintained as their main theme: effectiveness of the use of high-flow nasal cannula in improving the clinical picture in patients with Chronic Obstructive Pulmonary Disease (COPD). The primary sources were searched in the different databases and scientific portals -PubMed, Scielo and Google academic-; which were subsequently analyzed according to the Grade scale to determine their strength and quality of evidence. Results: Of the 10 articles reviewed, 10% (n=1/10) are meta-analyses, 10% (n=1/10) are systematic reviews, 10% (n=1/10) are multicenter randomized controlled trials , 20% (n=2/10) are Controlled Trials, 50% (n=5/10) are controlled and randomized trials. Conclusions: In 9 of the 10 articles (90%) it is evident that the use of high-flow nasal cannula is efficient in improving the clinical picture of patients with Chronic Obstructive Pulmonary Disease (COPD).

    Efficacy of conservative treatment in improving quality of life in patients with end-stage renal disease compared to renal replacement therapy

    Objective: Systematize evidence on the effectiveness of conservative treatment to improve quality of life in patients with end-stage renal disease compared to renal replacement therapy. Material and methods: Systematic observational and retrospective review, subjected to critical reading and using the GRADE evaluation system to identify the degree of evidence of the published articles. Of the 10 articles systematically reviewed, 40% were of high quality and 60% of moderate quality as described below: 40% (n= 4/10) are systematic reviews and 60% (n= 6/10) are studies Quasi-experimental cohort Results: 70% (n=7/10) indicate that conservative treatment is more effective than renal replacement therapy to improve quality of life. 30% (n=3/10) indicate that renal replacement therapy is more effective than conservative treatment in improving quality of life. Conclusion: 7/10 articles reviewed evidence that conservative treatment was more effective than renal replacement therapy to improve quality of life. 3/10 articles reviewed show that renal replacement therapy is more effective than conservative treatment in relation to: pain, discomfort, mobility, self-care, being less dependent and with fewer comorbidities

    Efficacy of Early Dual Antiplatelet Treatment in Stroke Patients

    Objectives: systematize evidence on the effectiveness of early treatment with dual antiplatelet therapy in patients with stroke. Methodology: Observational and retrospective systematic review, the search has been restricted to articles with full text and synthesizes the results of multiple primary investigations. They are an essential part of evidence-based medicine due to their rigorous methodology, identifying relevant studies. Results: In the final selection, 10 articles were chosen, we found that 70% (07) correspond to China, with 30% (03) we found Canada, USA, Korea. Most of the quasi-experimental studies have been studied, with 70 %, mainly in China. Conclusions: 80% affirm that dual antiplatelet treatment is effective in stroke. It was evident in the reviewed research that there is efficacy in the combination of acetylsalicylic acid and clopidogrel in the treatment of adult patients with stroke. It was shown to be effective in reducing risks and complications.

    Efficacy of hydrocolloid dressings in the treatment of pressure ulcers in intensive care unit patients

    Systematize the evidence on the effectiveness of the use of hydrocolloid dressings in the treatment of pressure ulcers in patients in the intensive care unit. Methodology: Systematic Review, 10 articles were chosen, which were studied, of which 70% are systematic reviews and meta-analysis, mainly in the countries of the United States and China, and 30% are cross-sectional analytical studies that were developed in the countries from Brazil, Spain and Peru Results: Of the articles studied, 70% show that products for the treatment of UPPs based on cure in a humid environment such as hydrocolloids have greater clinical efficacy than traditional treatment in the intensive care unit.Conclusion: The results of the review we have carried out show the existence of evidence in favor of the greater clinical effectiveness of dressings based on healing in a humid environment in the healing of pressure ulcers, lower overall cost and They are changed every three to five days, although they can remain in situ for up to seven days. And they are less effective than polyurethane foam (hydropolymer) dressings since they have a greater capacity for absorbing exudate and are easier to apply. removal than hydrocolloid dressings.
