Disnea percibida y tolerancia al ejercicio en pacientes con insuficiencia cardiaca estable en un centro fisioterapéutico, Lima 2026

Abstract

Introducción: La insuficiencia cardiaca es un síndrome clínico de alta prevalencia que se asocia a síntomas persistentes como la disnea y a una marcada limitación funcional, incluso en pacientes clínicamente estables. En el ámbito de la rehabilitación fisioterapéutica ambulatoria, la disnea percibida constituye un factor relevante que condiciona la tolerancia al ejercicio y la participación en programas de rehabilitación. Sin embargo, la evaluación estructurada de la disnea, considerando sus dimensiones físicas y afectivas, sigue siendo limitada en entornos fisioterapéuticos. Objetivo: Determinar la relación entre disnea percibida y tolerancia al ejercicio en pacientes con insuficiencia cardiaca estable en un centro fisioterapéutico de Lima, 2026. Material y métodos: Estudio cuantitativo, aplicado, correlacional y transversal. La población estará conformada por 80 pacientes con diagnóstico de insuficiencia cardiaca estable, seleccionados mediante censo. La disnea percibida se evaluará con el cuestionario Dyspnea-12, analizada globalmente y por dimensiones (física y afectiva), categorizada en tres niveles mediante percentiles. La tolerancia al ejercicio se medirá con el test de caminata de seis minutos (6MWT), clasificándose en baja, moderada y alta. El análisis estadístico incluirá estadística descriptiva y pruebas no paramétricas de asociación. Resultados: Se observó correlación negativa entre disnea percibida y tolerancia al ejercicio (ρ = -0,953; p < 0,001; N = 80). Según Cohen, la magnitud fue fuerte, indicando que mayor disnea se asocia con menor tolerancia. Conclusiones: Existe una relación significativa, negativa y fuerte entre disnea percibida y tolerancia al ejercicio en pacientes con insuficiencia cardiaca estable.

Introduction: Heart failure is a highly prevalent clinical syndrome associated with persistent symptoms such as dyspnea and marked functional limitation, even in clinically stable patients. In the context of outpatient physiotherapy rehabilitation, perceived dyspnea is a relevant factor that may condition exercise tolerance and patient participation in rehabilitation programs. However, structured evaluation of dyspnea, considering both physical and affective dimensions, remains limited in physiotherapy settings. Objective: To determine the relationship between perceived dyspnea and exercise tolerance in patients with stable heart failure at a physiotherapy center in Lima, 2026. Materials and Methods: A quantitative, applied, correlational, and cross-sectional study. The population will consist of 80 patients diagnosed with stable heart failure, selected through census sampling. Perceived dyspnea will be assessed using the Dyspnea-12 questionnaire, analyzed globally and by dimensions (physical and affective), and categorized into three levels using percentiles. Exercise tolerance will be measured with the six-minute walk test (6MWT), classified as low, moderate, or high. Statistical analysis will include descriptive statistics and non-parametric association tests. Results: A negative correlation was observed between perceived dyspnea and exercise tolerance (ρ = -0.953; p < 0.001; N = 80). According to Cohen’s criteria, the magnitude of this association was strong, indicating that patients with higher perceived dyspnea tended to show lower exercise tolerance. Conclusions: There is a significant, negative, and strong relationship between perceived dyspnea and exercise tolerance in patients with stable heart failure.
