Risk factors for severe preeclampsia and its association with preterm birth at the National Maternal Perinatal Institute 2017.

Abstract

Preeclampsia is a complex syndrome with a wide variety of clinical symptoms and varying gestational ages of onset. In Peru, it is the second leading cause of maternal death, accounting for 32%, and the leading cause of maternal death at the National Maternal Perinatal Institute, with 43%. Preeclampsia is a disease of unknown and multifactorial origin, with the definitive treatment being delivery. It also causes significant consequences for both the mother and the newborn. The most common risk factor for preeclampsia is multiparity. Along with maternal age >35 years, obesity, and maternal anemia, a significant association has been observed. For clinical purposes, preeclampsia must be diagnosed early, as the most important objective is the prevention of maternal and perinatal morbidity and mortality. This academic work presents a case of a pregnant woman at 34 weeks of gestation diagnosed with severe preeclampsia, with risk factors including multiparity, two prior cesarean sections, obesity, insufficient prenatal care, and a family history of hypertension. The patient presented with a complex clinical picture, suggesting the need for pregnancy termination by cesarean section after pulmonary maturation.

La preeclampsia es un síndrome complejo con una amplia variedad en la severidad de los síntomas clínicos y edad gestacional de inicio. en Perú es la segunda causa de muerte materna, con 32%, y la primera causa de muerte materna en el Instituto Nacional Materno Perinatal, con 43%. La preeclampsia es una enfermedad de origen desconocido y multifactorial cuyo tratamiento definitivo es el parto, además de ser causal de repercusiones sobre la madre y el recién nacido. El factor de riesgo más frecuente para preeclampsia es la multiparidad; este junto a una edad materna >35 años, obesidad, y anemia materna, mostraron una asociación significativa. Para fines clínicos, la preeclampsia se debe diagnosticar de manera precoz, porque el objetivo más importante es la prevención de la morbimortalidad materna y perinatal. Se presenta el trabajo académico de una gestante de 34 semanas de edad gestacional con diagnóstico de preeclampsia severa, con factores de riesgo de multiparidad, cesareada anterior dos veces, obesidad y control prenatal insuficiente y antecedentes familiares de hipertensión; presentando un cuadro clínico complejo; sugiriendo la interrupción del embarazo por cesárea previa maduración pulmonar.
