Effectiveness of antibiotic prophylaxis before incision or after cord clamping to reduce the risk of postoperative infectious complications in cesarean section patients

MATERIALS AND METHODS: This is a systematic review, its population being 150 scientific articles and its sample being 10 articles. Explorations were carried out in the databases of Cochrane Library, Embase, PubMed, Willey Online Library, Central database, Science Direct and Epistemonikos, to obtain results that compared the effectiveness of antibiotic prophylaxis prior to the incision and after clamping of the umbilical cord to reduce risks of infectious complications. Through the Grade system, the effectiveness of the evidence and the strength of recommendation of the articles were evaluated. RESULTS: 10 studies fully completed the insertion criteria. It was observed in 70% of research that antibiotic prophylaxis before incision is more effective than after clamping the umbilical cord, to reduce the risks of postoperative infectious complications. However, the remaining 30% argue that both antibiotic prophylaxis before of the incision compared to after clamping did not demonstrate superiority, that is, both are effective. CONCLUSIONS: Antibiotic prophylaxis before incision is more effective in preventing the emergence of infectious complications compared after clamping the umbilical cord in cesarean patients.
Resumen
MATERIALES Y METODOS: El presente es una revisión sistemática, siendo su población 150 artículos científicos y su muestra 10 artículos. Se realizó exploraciones en las bases de datos de Cochrane Library, Embase, PubMed, Willey Online Library, Central base de datos, Science Direct y Epistemonikos, para obtener resultados que compararan la eficacia de la profilaxis antibiótica previa de la incisión y posterior al pinzamiento del cordón umbilical para reducir riesgos en complicaciones infecciosas. A través del sistema Grade se evaluó la eficacia de evidencia y la fuerza de recomendación de los artículos. RESULTADOS: 10 estudios completaron en su totalidad los criterios de inserción. Se observó en el 70 % de investigaciones que la profilaxis antibiótica antes de la incisión es más eficaz que después del pinzamiento del cordón umbilical, para reducir los riesgos en complicaciones infecciosas postoperatorias, sin embargo, el restante 30% argumenta que tanto la profilaxis antibiótica antes de la incisión comparado con después del pinzamiento no demostró superioridad es decir ambas son eficaces. CONCLUSIONES: La profilaxis antibiótica antes de la incisión es más eficaz para evitar el surgimiento de complicaciones infecciosas comparado posterior al pinzamiento del cordón umbilical en las pacientes cesareadas.
