    Critical review: oral nutritional supplementation in the reduction of protein-energy wasting syndrome in patients with chronic kidney disease on dialysis

    The secondary research presented is a critical review titled "Oral Nutritional Supplementation in Reducing Protein-Energy Wasting Syndrome in Chronic Kidney Disease Patients on Dialysis." The objective was to analyze the effectiveness of oral nutritional supplementation (ONS) in reducing protein-energy wasting (PEW). The clinical question posed was: Will ONS reduce PEW in adults of both sexes with end-stage renal disease on dialysis? The Evidence-Based Nutrition (EBN) methodology was used, gathering scientific information from PUBMED and SCIENCE DIRECT, with 26 articles selected, and 11 of them being critically evaluated using the CASPe tool. The selected article, "Muscle State Response to Oral Nutritional Supplementation in Hemodialysis Patients with Protein-Energy Wasting: A Multicenter, Randomized, Open, Controlled Trial," has evidence level IA and a strong recommendation grade. The critical review concluded that ONS does reduce the prevalence of PEW in patients with end-stage renal disease undergoing dialysis.

    Diagnostic usefulness of the electrochemiluminescence method versus real-time PCR for hepatitis C diagnosis in a Lima dialysis center, 2018

    This study aimed to identify the diagnostic utility of the ECLIA assay method compared to the PCR-RT method for diagnosing Hepatitis C at a dialysis center in Lima in 2018. The study was observational, cross-sectional, and retrospective, analyzing results from dialyzed patients at a renal health center in Lima. The study included results from immunoserological tests using the ECLIA method and PCR-RT for patients with multiple etiologies, excluding samples with questionable or indeterminate immunoserological results, neonatal sera, and patients under 18 years old. The first detection method for HCV was ECLIA technology, and PCR-RT was used as the gold standard. Sensitivity and specificity, PPV, and NPV were calculated to determine the validity and diagnostic security of the ECLIA method compared to PCR-RT.Of 123 patients with results from both tests between January and December 2018, 45% were reactive by ECLIA (55/123) and 17% were detectable by PCR-RT (21/123). Sensitivity was 90% (19 reactive by ECLIA out of 21 detectable by PCR-RT), specificity was 65% (66 non-reactive by ECLIA out of 102 non-detectable by PCR-RT), PPV was 93%, and NPV was 57%. The Cohen Kappa index was 0.34, with a concordance level of 69.1% between both tests, excluding random coincidences. The study concluded that the ECLIA method is highly sensitive and moderately specific for HCV in dialyzed patients, with a high degree of diagnostic security for confirming both sick and healthy patients. The degree of concordance in the diagnosis between ECLIA and the gold standard was moderate.

    Effectiveness of kidney transplant compared to dialysis in improving the quality of life in patients with chronic kidney failure

    Materials and Methods: The type of study of this work is qualitative, with a design of systematic reviews, controlled clinical trials, cross-sectional analytical, correlational and quasi-experimental study, the database instrument Pubmed NCBI, scielo, Cochrane. Not older than 10 years, in English. Results: in the present study, a population of 20 research articles was analyzed, the sample was 10 articles, of which 40% (4) correspond to systematic review, 30% (3) controlled clinical trials, 10% (1 ) cross-sectional analytical, 10% (1) correlational, 10% (1) is quasi-experimental study. We find that 30% (03) correspond to Spain, with 20% (02) we find Poland and with 10% (01) we find Canada, the United States, France, Georgia and the United Kingdom. Conclusions: According to the 10 articles reviewed, 100% conclude that kidney transplant is more effective and provides better quality of life because they experience better life participation rates compared to patients who receive dialysis, and the risk of mortality and cardiovascular events is reduced. The emotional role of mood in post-transplant patients significantly improves the quality of life and is classified as excellent, this is due to the fact that the majority of patients have a support network of their family, friends and/or healthcare professionals. health.

    Efficiency of descending conductivity and ultrafiltration profiles in renal patients

    Objective: Systematize the available evidence on the efficiency of descending conductivity and ultrafiltration profiles in renal patients on hemodialysis. Methodology: This is a Systematic Review of 10 articles in the MEDLINE, PUBMED, LILACS, SCIELO database, whose search has been restricted to articles with full text, and the selected articles were subjected to critical reading, using the Sackett evaluation. to identify its level of evidence. Results: With the use of descending conductivity and ultrafiltration profiles, renal patients on hemodialysis have better tolerance to the treatment, their application reduces intradialytic complications such as hypotension, cramps, nausea, vomiting and headaches, and also allows the patient to complete in a stable manner. its treatment time and helps it maintain an adequate dry weight since it is possible to sequester all the accumulated volume. All of this leads to the patient reaching their ideal dialysis dose goal, making hemodialysis a safe and effective treatment that benefits their quality of life. Conclusion: In light of the evidence, the effectiveness of descending conductivity and ultrafiltration profiles in improving the care of renal patients is concluded by verifying that they reduce intradialytic complications, help maintain the patient's dry weight and improve morbidity and mortality rates. in hemodialysis treatment, improving their quality of life.

    Level of knowledge about self-care of arteriovenous fistula in patients at the San Juan Masías Dialysis Center, Lima – 2020

Metodología: Será un estudio de enfoque cuantitativo, de tipo aplicada, de diseño no experimental, de nivel correlacional de corte transversal, la población y muestra estuvo conformada por 100 pacientes, para la recolección de datos se utilizó la técnica la encuesta para la primera variable y para la segunda variable, la escala tipo Likert.

    This research aims to: determine the relationship between the quality of care offered by nursing staff and the satisfaction of patients undergoing dialysis treatment. The methodology: we will work with a quantitative nature, of applied type, with a design that is developed in the nonexperimental with a cross section, also work with a sample of 90 patients who are treated at the Francisco I Medical Support Center, the instruments that will be used for the research work will be; an observation guide to evaluate the quality of care developed by the author Barreda and a questionnaire for measuring patient satisfaction authored by Castro y Cadena, instruments that are validated and reliable, where their application will last approximately 30 minutes, with which it will be possible to obtain a database that will be processed statistically both at the descriptive and inferential level, descriptive because it will present variables and dimensions in tables and figures, and inferential because through Spearman's Rho test will proceed with the verification of the hypotheses of studies, and based on these results will proceed with the conclusions and recommendations of the case.

    Quality of life and functional dependence in older adult patients undergoing hemodialysis in the nephrology service of the Guillermo Almenara Irigoyen Hospital – 2020

    Chronic kidney disease is a multifactorial pathophysiological process of gradual and irreversible condition that leads to a terminal state, where the patient demands renal replacement therapy, that is, dialysis or transplant in order to survive. Elderly patients on hemodialysis have greater associated comorbidity and worse quality of life, associated with a high degree of dependency. The objective of this study is to determine the relationship between the level of quality of life and functional dependence in older adult patients undergoing hemodialysis at the Guillermo Almenara Irigoyen Hospital - 2020. The present research is quantitative, non-experimental, cross-sectional, descriptive and correlational. . The population and sample will be made up of 84 elderly patients with functional dependence undergoing hemodialysis in the Nephrology service during the months of July to September 2020. The technique for data collection is the survey and the instrument is two questionnaires: SF - 36 that evaluates quality of life, and the Barthel Index that evaluates functional dependence.

    Quality of life and self-care in patients with end-stage chronic kidney disease on dialysis at a Los Laureles Support Center San Juan de Lurigancho, 2022

    In the world, it is estimated that CKD affects 850 million people, with progressive and irreversible damage to kidney function requiring renal replacement therapy. This public health problem is the sixth cause of death, reaching figures of 2.4 million deaths per year, while 13 million people are affected by acute kidney injury. In Peru, a serious public health problem is CKD, which according to 2021 estimates, around 3,060,794 million people over 18 years of age would be affected by this disease in stages I to IV. Given this, the objective of the research is "Relate quality of life and self-care in patients with end-stage chronic kidney disease on dialysis at a Los Laureles support center, San Juan de Lurigancho, 2022." The methodology applied in this research is hypothetical deductive with a quantitative approach of an observational type and a descriptive correlational design. The study population is made up of 320 patients who receive hemodialysis at the Laureles support center and 175 were considered as part of the sample which will be non-probabilistic and for convenience. The instruments for obtaining the research data will be two valid and reliable questionnaires called “Self-care of the adult patient in hemodialysis treatment 2016” and “Quality of Life” (Cronbanch's alpha 0.935 and 0.886 respectively). The first was created by Cárdenas G. which consists of 21 items grouped into three dimensions such as social interaction, activity and rest, and food consumption. The second instrument was developed by Vallejos N. and consists of 32 items grouped into physical, emotional and social dimensions.

    Revisión crítica: efecto de la infusión de hierro parenteral en los niveles de hemoglobina en pacientes adultos con enfermedad renal crónica en diálisis

    La anemia ferropénica y la disminución de producción de eritropoyetina son comunes en la enfermedad renal crónica. Existen diferentes estrategias de tratamiento, como es la infusión parenteral de hierro. El objetivo de esta investigación Revisión crítica: Efecto de la infusión de hierro parenteral en los niveles de hemoglobina en pacientes adultos con enfermedad renal crónica en diálisis, fue conocer la respuesta a la interrogante clínica: ¿Cuál es el efecto de la infusión de hierro parenteral en los niveles de hemoglobina en pacientes adultos con Enfermedad Renal Crónica en Diálisis? Aplicamos el método Nutrición Basada en Evidencia (NUBE). Para buscar información se utilizó MEDLINE, SCIELO, SCOPUS, PUBMED, identificado 92 artículos, siendo elegidos 11, los cuales fueron evaluados con la herramienta CASPE, siendo seleccionado al final el Ensayo Clínico Aleatorio titulado como: Maintenance intravenous iron in hemodialysis patients to minimize erythropoietin doses: a double blinded, randomized controlled trial (the MAINTAIN IRON trial), que tiene una evidencia AI y una Recomendación “FUERTE”. El juicio crítico permitió concluir que estos pacientes es importante suplementar hierro en la ERC, en especial durante la diálisis.

    Revisión crítica: efecto de la suplementación nutricional en el peso corporal de adultos con ERC en diálisis

    La ERC es prevalente entre los peruanos adultos, además se ha observa que el consumo de suplementos nutricionales podría tener un impacto significativo en el peso corporal y ser una herramienta valiosa para mejorar el estado nutricional generando cambios positivos en paciente adultos con ERC. Este trabajo, que corresponde a una investigación secundaria bajo el enfoque de revisión crítica, se tituló: Efecto de la suplementación nutricional en el peso corporal de adultos con ERC en diálisis, tuvo como objetivo identificar artículos de investigación que puedan responder la pregunta clínica fue: ¿cuál es el efecto de la suplementación nutricional en el peso corporal de adultos con ERC en diálisis? Para ello, se utilizó la metodología Nutrición Basada en Evidencia (NuBE). Realizando la búsqueda de estudios científicos en bases de datos como BMED, MEDLINE, SCIELO, SCIENCE DIRECT, MEDIGRAPHIC, DOAJ, MDPI, WILEY ONLINE LIBRARY, HINARI, CJASN, SCIENCE ALERT encontrando 28 artículos, identificados inicialmente, se seleccionaron 10 mediante una evaluación crítica utilizando CASPE, eligiendo el Ensayo Clínico Aleatorizado titulado como Glycine increases fat-free mass in malnourished haemodialysis patients: a randomized double-blind crossover trial, el cual posee un nivel de evidencia A I y Grado de Recomendación Fuerte, según el Juicio del investigador. La rreflexión crítica permitió concluir que la suplementación con glicina durante 4 meses mejoro significativamente la masa libre de grasa generando un aumento de peso corporal en los pacientes adultos con HD.

    Revisión crítica: efectos del ejercicio físico en la capacidad funcional en pacientes con enfermedad renal crónica

    Los pacientes con enfermedad renal crónica (ERC) son físicamente inactivos y presentan un deficiente rendimiento; lo que implica una mayor morbilidad y mortalidad, así como en una reducción de la velocidad en la que los riñones filtran la sangre y un consiguiente deterioro de su calidad de vida. La investigación secundaria titulada “Revisión crítica: Efectos del ejercicio físico en la capacidad funcional en pacientes con enfermedad renal crónica”, tuvo como objetivo revisar evidencia sobre ejercicio físico y su efecto en los resultados clínicos en estos sujetos. La pregunta clínica formulada fue: ¿De qué manera impacta el ejercicio físico en la capacidad funcional en personas con deterioro renal crónico? Se empleo la estrategia NuBE. La recopilación de datos se llevó a cabo en SCOPUS, PUBMED, MEDLINE, SCIELO, DIMENSIONS, JURN hallando 80 publicaciones, eligiendo 33 de ellos y examinados mediante la herramienta CASPE, seleccionándose finalmente un ensayo clínico aleatorizado titulado: “Comparación de los efectos de 4 meses de dos programas de entrenamiento físico autoadministrados sobre el rendimiento físico en pacientes con enfermedad renal crónica: RENEXC: un ensayo controlado aleatorio”, presenta una base cientifica de nivel A I y una recomendación categorizada como FUERTE, justificada en la pericia del investigador. El comentario crítico determino que realizar entrenamiento físico de intensidad moderada por 150 minutos semanales mejora la resistencia general y muscular, la fatigabilidad, la motricidad fina, el equilibrio y la capacidad cardiorrespiratoria.

    Self-care and quality of life in hemodialysis patients, Francisco I medical support center, San Juan de Lurigancho, San Juan de Lurigancho-2022

    Objective: determine the relationship that exists between self-care and quality of life in hemodialysis patients, at the Francisco I Medical Support Center in San Juan de Lurigancho- 2022. Material and methods: method: hypothetical deductive, approach: quantitative, type : correlational, design: Non-experimental cross-sectional. The sample was 90 patients on hemodialysis. The Instrument to measure the self-care variable: the Huaylinos et al. questionnaire was used. Which is made up of 22 items, distributed in 5 dimensions. With Validation, it was through the judgment of 3 experts, giving an agreement of 98% of applicability with a reliability, an α of 0.86 was obtained. The Instrument to measure the Quality of Life Variable: was structured by Vallejos in Peru in 2019. It consists of a total of 32 items with 3 dimensions, 97% agreement of applicability, in reliability an α of 0.811 was obtained. Results: it was found that 20% had poor self-care, 52.2% had regular self-care, and 27.8% had good self-care. On the other hand, 13.3% had a low quality of life, 53.3% had an average quality of life, and 33.3% had a poor quality of life. quality of life was high.

    The objective of this project is to determine the relationship between self-care and quality of life of patients with chronic kidney disease. Descriptive hypothetical-deductive method study, quantitative approach, applied type, with cross-sectional correlational design, its population will be patients with chronic kidney disease at Centro de Dialisis Interdial Norte S.A.C, with the sample of 178 patients chosen by random sampling not simple probabilistic. The technique to be used will be the survey, the instrument being two questionnaires, one for self-care and the other for quality of life. It will be coordinated with the Dialysis Center by means of a cover letter requesting authorization to apply the survey. Data processing and analysis will be obtained using the statistical package SPSS.

The instrument to be utilized is the Self-Care Agency Scale (ASA), based on Dorothea Orem's theory, consisting of 24 items rated using the Likert scale, grouped into 6 dimensions. The study population will include a total of 200 patients, of which 120 are undergoing hemodialysis and 80 peritoneal dialysis at the hospital, who meet the eligibility criteria and agree to participate in the study by signing the informed consent.

    Social support in patients of a particular dialysis center belonging to a national hospital – 2020.

    The objective of this study is to determine social support in patients attending a private dialysis center affiliated with a national hospital. The research is descriptive, with a quantitative approach and a cross-sectional design, considering a population of 60 patients receiving hemodialysis treatment at a private dialysis center. The MOS social support questionnaire will be applied to them. This research project will be presented to the Ethics Committee of the Peruvian University of Norbert Wiener. Subsequently, the data collection process will begin, following administrative arrangements with the dialysis center to agree on the date and time for administering the instrument. The collected information will first be entered into Microsoft Excel 2018, then exported to IBM SPSS Statistics version 26.0, where the results will be interpreted through tables and graphs, which will then be described and discussed in the results and discussion section, adhering to ethical principles.
