    Cardiovascular risk and body mass index in hypertensive patients to a cardiology clinic in a health facility in Surquillo- 2019

    Objective: To determine the cardiovascular risk and body mass index in hypertensive patients attending a cardiology clinic in a health establishment in Surquillo - 2019. Material and method: The study has a quantitative approach, with a non-experimental, descriptive, and cross-sectional design, including a population of 125 hypertensive patients aged 40 to 80 years, attending a cardiology clinic in a health establishment in Surquillo. The data collection technique was a survey, and the data collection instruments were the Cardiovascular Risk Calculator developed by the PAHO and the Body Mass Index from the WHO. Results: Regarding cardiovascular risk, the predominant risk was low with 52.8%, followed by 34 patients with moderate risk (27.2%), 16 patients with high risk (12.8%), 6 patients with critical risk (4.8%), and finally, 3 patients with very high risk (2.4%). As for body mass index, 58 patients were overweight (46.4%), followed by 50 patients with healthy weight (40%), 13 patients with moderate obesity (10.4%), 3 patients with severe obesity (2.4%), and finally, 1 patient with underweight (0.8%). Conclusions: Regarding cardiovascular risk, low risk predominated, followed by moderate, high, critical, and very high risks. Regarding body mass index, overweight predominated, followed by healthy weight, moderate obesity, severe obesity, and underweight.

    Characterization of patients who abandoned pulmonary tuberculosis treatment at Infantas Health Center, San Martín de Porres.

    Objective: To determine the correlation between school climate and bullying among adolescents at the José del Carmen Marín Arista Educational Institution in Lima Metropolitan Area – 2017. Materials and Methods: This was a quantitative study, with a non-experimental, descriptive, and correlational design. The study population consisted of 615 students, from which 310 met the inclusion and exclusion criteria. The instruments used were the School Social Climate Scale (CECSCE) developed by Trianes et al., and the Adolescent Peer Relations Instrument – Bullying (APRI-Bullying), developed by Parada et al. Results: Of the total adolescents evaluated, 58.4% were male and 41.6% female. The majority were aged between 13 and 14 years. Regarding the grade levels, 23.5% (73) were in the third year, and 23.2% (72) were in the first year. The perception of the school climate was deemed inadequate by most (51%, n=158). Bullying was moderate for 41% (n=127) and severe for 38% (n=118). Conclusion: A statistically significant inverse correlation was found between school climate and bullying in adolescents at the José del Carmen Marín Arista Educational Institution.

    This study is of a quantitative approach, with a non-experimental, descriptive, and correlational design. The population consisted of 615 students, and after applying the inclusion and exclusion criteria, 310 students remained. Conclusion: There is a statistically significant, slightly inverse relationship between school climate and bullying among adolescents at the I.E. 1035 José del Carmen Marín Arista.

    EFECTIVIDAD DE LA ADMINISTRACIÓN DE HIERRO ENDOVENOSO VERSUS ORAL EN EL TRATAMIENTO DE LA ANEMIA EN PACIENTES CON ENFERMEDAD RENAL CRÓNICA

    Analizar y sistemizar sobre la efectividad de la administración de hierro endovenoso versus oral en el tratamiento de la anemia en pacientes con enfermedad renal crónica. Material y Métodos: Revisión sistemática observación y retrospectiva de patrón cuantitativa, puesto a elección critica, realizando el sistema de evaluación Grade para la identificación del nivel de evidencia, encontrados bases de datos: PubMed, Sciencedirect, Scielo, Wiley Online Library, Elsevier, Epistemonikos .De los 10 artículos revisados sistemáticamente el 40% (n= 4/10) son verificaciones sistemáticas, el 50% (n= 5/10) son ensayos aleatorizados controlado y el 10 % (n= 1/10) son análisis de cohorte. En base a resultados encontrados en la exploración sistemática elaborada en este estudio, derivado de los países de Israel (20%), seguida de Australia (20%), Inglaterra (20%), EEUU (20%), Italia (10%) y Nepal (10%). Resultados: De este modo de las evidencias halladas, el 100% (n=10/10) señalan que la administración de hierro endovenoso es más efectiva que la oral en el tratamiento de anemia en pacientes con enfermedad renal crónica. Conclusión: La administración de hierro endovenoso es más efectivo que la administración vía oral en el tratamiento de la anemia en pacientes con patología renal crónica

    Effectiveness of noradrenaline versus dopamine in patients with septic shock that come to the emergency service

    Objective: To analyze and systematize the evidence on the efficacy of dopamine versus norepinephrine for patients with septic shock in the emergency department. Materials and methods: This study is a systematic review. The GRADE system was used to assess the quality of the evidence and grade the strength of the recommendations. The search was conducted in the following databases: LILACS, SciELO, Cochrane Plus, PubMed, MEDLINE, Epistemonikos, and EBSCO. Of the 10 articles systematically reviewed, 30% were systematic reviews and meta-analyses, 20% were clinical trials, and 20% were randomized primary studies. Regarding the country of origin, 30% were from the U.S. and China, and 10% were from France, India, England, and Spain. Results: 90% of the articles agree that norepinephrine is more effective than dopamine, as it has a lower incidence of cardiac-related events compared to dopamine, and 10% found no difference in mortality between the two drugs. Conclusions: It is concluded that norepinephrine is more effective than dopamine in patients with septic shock in the emergency department.

    Effectiveness of reminiscence therapy in reducing depression among elderly patients.

    This work aims to assess the effectiveness of reminiscence therapy in reducing depression in older adult patients. This study used a quantitative, observational, and retrospective systematic review method, subject to a critical classification, using the GRADE evaluation system to recognize the level of evidence, found in the following databases: PubMed, Epistemonikos, ScienceDirect, ResearchGate. Thirty articles were reviewed, of which 10 original articles were selected, 20% (n = 2/10) are meta-analyses, 10% (n = 1/10) is a systematic review, 30% (n = 3/10) are randomized controlled trials, and 40% (n = 4/10) are quasi-experimental studies. According to the results obtained from the systematic review conducted in this study, the articles come from the countries of China (70%), followed by Spain (20%) and Australia (10%). Results: Furthermore, of the evidence found, 100% (n = 10/10) indicate that reminiscence therapy reduces depressive symptoms and cognitive functions in older adults. Conclusion: Reminiscence therapy is effective in reducing depression in older adult patients.

    Effectiveness of the harmonic scalpel for controlling intraoperative bleeding in patients undergoing surgery

    Objective: To systematize the evidence on the effectiveness of the harmonic scalpel for controlling intraoperative bleeding in patients undergoing surgical intervention. Materials and Methods: This is an observational and retrospective systematic review that synthesizes the results of multiple primary studies, which are an essential part of evidence-based nursing due to their rigorous methodology, identifying relevant studies to answer specific clinical practice questions. The search was restricted to full-text articles, and the selected articles underwent critical reading using the GRADE system to assign the strength of recommendations. Results: The studies were found in the following databases: Lilacs, Scielo, Pubmed, Medline, Ebsco. All studies correspond to the following study types and designs: systematic review and meta-analysis (3), meta-analysis (4), controlled clinical trial (1), randomized clinical trial (1), and meta-analysis of randomized clinical trials (1). 100% (n=10/10) of the studies evidence that the use of the harmonic scalpel is highly effective in controlling intraoperative bleeding during surgical interventions. Conclusions: All the scientific articles included in this systematic review provide evidence that the harmonic scalpel is effective in controlling intraoperative bleeding in patients undergoing surgical interventions. It should be emphasized that the evidence supporting this conclusion comes from studies of the highest level regarding the quality of evidence and strength of recommendation, and therefore, the use of the harmonic scalpel should be implemented in all surgical interventions.

    Effectiveness of the use of probiotics for the prevention of necrotizing enterocolitis and mortality in preterm infants

    Objective: To systematically analyze the evidence on the efficacy of probiotics for the prevention of necrotizing enterocolitis and mortality in preterm newborns. Material and method: Eleven scientific articles on the efficacy of probiotics for the prevention of necrotizing enterocolitis and mortality in preterm newborns were found in the databases Cochrane, Scielo, Lilacs, and Medline/Pubmed. A systematic review was applied, analyzing the studies using the GRADE scale to verify their strength and quality of evidence. Of the 11 articles reviewed, 36% (4/11) correspond to the methodological design of systematic reviews, 55% (6/11) are meta-analyses, and 9% (1/11) is a systematic review and meta-analysis. Results: 82% (9/11) demonstrate the efficacy of probiotics for the prevention of necrotizing enterocolitis and mortality in preterm newborns, and 18% (2/11) show that probiotics prevent only necrotizing enterocolitis. Conclusions: From the 11 articles reviewed, the efficacy of probiotics for the prevention of necrotizing enterocolitis and mortality in preterm newborns is evidenced.

    Objective: To systematize the evidence on the effects of work overload on the job performance of nursing staff. Materials and Methods: An observational and retrospective systematic review of a quantitative nature was conducted, subjected to critical selection, using the GRADE evaluation system to identify the level of evidence, found in the following databases: PubMed, Sciencedirect, Scielo, Wiley Online Library, Elsevier, Epistemonikos. Of the 10 articles reviewed, 40% are bibliographic reviews, 30% are descriptive studies, 20% are qualitative studies, and 10% are literature reviews. According to the results obtained from the systematic review conducted in this study, the articles come from Brazil (20%, n=2/10), Chile, Colombia, and Peru (10%, n=1/10). Results: Of the evidence found, 100% (n=10/10) indicate that work overload negatively affects the job performance of nursing staff. Conclusion: It is concluded that the effects of work overload influence the job performance of nursing staff, reducing productivity, leading to lower performance, causing serious physical and mental health problems, and negatively affecting their efficiency and quality of life, thereby decreasing their professional performance.

    Efficacy of aggressive intravenous hydration versus standard intravenous hydration in the treatment of acute pancreatitis.

    Objective: To analyze the evidence on the efficacy of aggressive intravenous hydration versus standard intravenous hydration for the treatment of patients with acute pancreatitis. Methodology: The design was a systematic review that summarized the results and evidence from 14 studies, which was an essential part of Evidence-Based Nursing, highlighting those studies that were relevant to answer specific clinical practice questions. Data were collected through a literature review of national and international research articles addressing intravenous hydration treatment for acute pancreatitis. Articles published before 2011 were excluded. The databases used were Elsevier, Scielo, and NCBI. Results: In 21.4% of the articles (3/14), there were controversies regarding both types of hydration. Some studies support aggressive intravenous hydration, while others do not; however, aggressive intravenous hydration was recommended in the initial stage in 64.3% of the studies (9/14), while standard intravenous hydration was recommended in 14.3% of the studies (2/14) as it was less harmful and improved patient recovery. Conclusions: Aggressive intravenous hydration was the most recommended, provided it was applied in the early or initial stage of the disease or in patients with mild or moderate pancreatitis.

    Efficacy of an exercise program to prevent falls in older adults living in the community

    Materials and Methods: A systematic review of international research articles was conducted, obtained through searches in databases: Pubmed, Epistemonikos, Sciencedirect, Cochrane, Researchgate, which were selected and analyzed using the GRADE system to determine the respective degree of evidence. The search for articles was carried out using the DECS and MESH descriptors for searches in Spanish and English. The systematic review consists of 10 identified scientific articles, of which 40% (4/10) are meta-analyses, 20% (2/10) are systematic reviews, and 40% (4/10) are randomized controlled trials. Regarding the origin of the articles, 20% (2/10) are from Australia, and 80% (8/10) are from France, South Korea, the United States, Malaysia, Germany, Japan, Switzerland, and Spain, respectively. Results: 100% (10/10) indicate that exercise programs are effective in preventing falls in elderly individuals living in the community. Conclusions: It was found that 10 out of 10 (10/10) of the studies concluded that an exercise program is effective in preventing falls in older adults living in the community.

    Efficacy of nebulized furosemide for outpatient treatment of adult patients with asthma.

    Materials and Methods: The method is a systematic review, which simplifies the results of multiple primary investigations, using the GRADE system to identify the quality of evidence and the strength of recommendations. Results: According to the evidence from the studies, 90% (n=09/10) of the articles demonstrate that nebulizations with furosemide added to saline solution are effective in reducing bronchospasm in mild and moderate acute bronchial asthma. Similarly, 10% (n=01/10) of the studies found in the evidence on the effectiveness of furosemide used in nebulization suggest that, due to the relevance of the results, it cannot yet be considered an asthma medication, and it is recommended to conduct further studies with larger sample sizes. The studies in the systematic review were conducted in Cuba, Honduras, the United Kingdom, the United States, Spain, Mexico, and Italy. Conclusions: It is concluded that the use of furosemide in nebulization is effective for the outpatient treatment of adult patients with asthma.

    Objective: To systematically analyze evidence regarding the efficacy of furosemide nebulization in reducing bronchospasms in patients with acute mild and moderate bronchial asthma. Materials and Methods: Systematic review methodology, synthesizing results from multiple primary studies. The GRADE system was used to assess evidence quality and recommendation strength. Results: 90% (n=09/10) of the reviewed articles demonstrate that nebulization with furosemide added to saline solution is effective in reducing bronchospasms associated with mild and moderate acute bronchial asthma. 10% (n=01/10) of the studies highlight the need for further research with larger sample sizes to conclusively establish furosemide as an antiasthmatic medication. Studies included in this systematic review originated from Cuba, Honduras, the United Kingdom, the United States, Spain, Mexico, and Italy. Conclusions: Furosemide nebulization is effective for outpatient treatment of mild and moderate bronchial asthma in adult patients. Further research with broader sample sizes is recommended to strengthen the evidence base and clarify its antiasthmatic potential.

    Efficacy of renal transplantation in decreasing the risk of graft loss for survival in pediatric patients.

    Objective: To systematize and evidence the efficacy of kidney transplantation in the reduction of the risk of graft loss for survival in pediatric patients. Materials and Methods: The study is quantitative, with a systematic review design. The population consisted of 39 articles that were reviewed, and 10 scientific articles were selected as the sample. Of these, 20% (n=2/10) were systematic reviews, 10% (n=1/10) were systematic review and meta-analysis, and 70% (n=7/10) were cohort studies. According to the quality of evidence, 30% was rated as high and 70% as moderate. In terms of the strength of recommendations, 30% were strong and 70% were weak, based on the country where the research was conducted. Results: The reviewed articles show that 40% (n=4/10) indicate the efficacy of renal replacement therapy in the risk of graft loss for survival in pediatric patients. Another 40% (n=4/10) state that it is partially effective, and 20% (n=2/10) state it is not effective. Conclusion: It is concluded that 40% (n=4/10) indicates that renal replacement therapy is partially effective in the risk of graft loss for survival in pediatric patients.

    Efficacy of the use of probiotics for the prevention of necrotizing enterocolitis and mortality in premature newborns.

    Objective: Systematize the evidence on the effectiveness of oral lactoferrin for the prevention of necrotizing enterocolitis and sepsis in preterm newborns. Materials and Methods: This systematic review consists of 10 scientific articles found on the effectiveness of oral lactoferrin for the prevention of necrotizing enterocolitis and sepsis in preterm newborns, found in the following databases: Lipecs, Lillacs, Scielo, Chrocane, Medline, and Pubmed. Among the evidence, we found experimental research designs, meta-analyses, and systematic reviews, with the articles published in Spanish and English, and no older than ten years. Results: Of the 10 articles reviewed, 90% (9/10) show that the administration of oral lactoferrin in preterm newborns, with very low birth weight and neonates, decreases late-onset sepsis and necrotizing enterocolitis. 10% (1/10) does not show the effectiveness of oral lactoferrin in preterm newborns. Conclusion: The effectiveness of oral lactoferrin for the prevention of necrotizing enterocolitis and sepsis in preterm newborns is evident.
