    Desempeño laboral y productividad laboral en el personal del departamento de la unidad de cuidados intensivos en un Hospital – Lima, 2024

    Objetivo: Determinar la relación entre el desempeño laboral y la productividad laboral en el personal del departamento de la UCI de un Hospital en Lima, 2024. Materiales y métodos: Se realizó un estudio cuantitativo, de tipo aplicado, con diseño no experimental, transversal y correlacional. La población censal estuvo conformada por 102 profesionales de la salud, a quienes se aplicaron cuestionarios validados y confiables sobre desempeño laboral y productividad laboral. Resultados: Se encontró una correlación positiva y moderada entre el desempeño laboral y la productividad laboral (Rho=0,495; p=0,000). Además, cada una de las dimensiones del desempeño laboral mostró correlación significativa con la productividad laboral (p<0,05). Conclusión: Existe relación significativa entre el desempeño laboral y la productividad laboral en el personal de la UCI en el Hospital estudiado.

    Effective oral hygiene for the prevention of pneumonia in patients with mechanical ventilation

    Objective: To analyze and systematize evidence on the effectiveness of oral care in preventing pneumonia in patients receiving mechanical ventilation. Materials and Methods: This is a systematic review of evidence on the effectiveness of oral care in preventing pneumonia in patients with mechanical ventilation. Among all the articles found, the most significant ones were included based on their level and quality of evidence, while less relevant ones were excluded. The databases used for the review were Scielo, Medplus, PubMed, and BVS. Results: A total of 10 articles were selected from different countries, with 50% (5) being systematic reviews, 20% (2) cohort studies, 20% (2) randomized controlled trials, and 10% (1) a meta-analysis. Of the articles analyzed, 70% (n=7/10) confirm that oral care using only oral solutions or mouthwashes, specifically chlorhexidine, is effective in preventing pneumonia in patients receiving mechanical ventilation. The remaining 30% (n=3/10) suggest that oral care is effective not only with chlorhexidine mouthwashes but also by adding other strategies and solutions to prevent pneumonia caused by mechanical ventilation. Conclusion: The reviewed studies are conclusive in stating that oral care for ICU patients on mechanical ventilation is effective in preventing pneumonia, particularly when using oral antiseptics such as 2% chlorhexidine. These measures should be included as part of the overall care for mechanically ventilated patients upon admission to prevent bacterial colonization that can lead to pneumonia.

    Effective preventive measures applied by nurses to prevent ventilator-associated pneumonia in intensive care units

    Objective: To determine effective preventive measures taken by nurses to prevent ventilator-associated pneumonia in the ICU. Materials and Methods: A systematic review of four articles was conducted. Results: The most effective preventive measures include head elevation, secretion aspiration, oral hygiene, monitoring cuff pressure, and measuring gastric residual volume. Conclusion: Elevating the head of the bed and other preventive measures are essential in reducing ventilator-associated pneumonia in ICU patients.

    Effective strategies to strengthen growth and development in children

    Objective: Systematize the best evidence in already completed research; That is, to what extent the research found adds new knowledge to the central question. Material and methods: Information was collected from different articles according to the level of evidence and degree of recommendation. Thus, descriptive and correlational studies were selected. Results: The factors that intervene in the non-attendance of the CRED control of the boy and girl, the mothers and/or caregivers report that the nursing staff is insufficient and they have difficulties with the hours of care, with the waiting time, diversity of tasks of the home. Conclusions: It is detailed that in the range of 4 to 8%, fathers, mothers, caregivers have difficulties taking their children to the CRED due to work schedules, leaving their children in the care of minors, due to ignorance of the importance of the CRED, generating a significant gap between the children who attend the CRED and those who do not attend, which could be decisive, so that the children do not reach optimal development, causing them to be limited in their cognitive development and school performance, limiting their possibilities to access other levels of education, and with low economic productivity when they reach the adult stage of life. This is why it is essential to generate effective strategies to strengthen the growth and development of children; it is necessary to involve the person, family and community in this context, to preserve family health in the population.

    Effectiveness of ATP bioluminescence to evaluate surface cleanliness in healthcare establishments

    Objective: To analyze and systematize the effectiveness of ATP bioluminescence for evaluating surface cleanliness in healthcare facilities. Materials and Methods: A systematic, observational, and retrospective review of a quantitative nature, subjected to critical selection, using the GRADE evaluation system to identify the level of evidence, found in the following databases: PubMed, Sciencedirect, Scielo, Wiley Online Library, Elsevier, and Epistemonikos. Of the 10 articles systematically reviewed, 30% (n=3/10) are systematic reviews, and 70% (n=7/10) are cohort studies. These studies come from Italy (30%), followed by Brazil (10%), China (10%), Sweden (10%), Canada (10%), Chile (10%), the USA (10%), and the UK (10%). Results: Of the evidence found, 60% (n=6/10) (16, 17, 22, 23, 24, 25) indicate that ATP bioluminescence is effective for evaluating surface cleanliness in healthcare facilities. 40% (n=4/10) (18, 19, 20, 21) indicate that ATP bioluminescence is not effective for evaluating surface cleanliness in healthcare facilities. Conclusion: ATP bioluminescence is effective for evaluating surface cleanliness in healthcare facilities.

    Effectiveness of bee honey in the treatment of acute cough to decrease its severity, frequency and improve the quality of sleep in children from 1 to 5 years old

    Objective: Systematize the evidence on the effectiveness of honey in the treatment of acute cough to reduce severity, frequency, and improve sleep quality in children aged 1 to 5 years. Material and Method: Observational and retrospective systematic review, the search was restricted to 10 full-text articles. The reviews of the articles were obtained from the following databases: Pubmed, Medline, Embase, Cochrane Library, Dialnet, Scielo, and Lilacs. They were analyzed using the Grade scale to determine the strength and quality of the evidence. Of the 10 articles, 70% (7/10) were randomized clinical trials (RCTs), 10% (1/10) were systematic reviews, 10% (1/10) were experimental studies, and 10% (1/10) were cross-sectional studies. The articles came from Canada 20% (2/10), Saudi Arabia 10% (1/10), Iran 10% (1/10), Nigeria 10% (1/10), Malaysia 10% (1/10), Italy 10% (1/10), Chile 10% (1/10), Egypt 10% (1/10), and Brazil 10% (1/10). Results: Of the 10 articles reviewed, 100% (10/10) showed that honey is effective in the treatment of acute cough to reduce severity, frequency, and improve sleep quality in children aged 1 to 5 years. Conclusions: The articles, 100%, show that honey is effective in the treatment of acute cough to reduce severity, frequency, and improve sleep quality in children aged 1 to 5 years, by boosting the immune system, with antimicrobial, antibacterial, and anti-inflammatory effects.

    Effectiveness of early ambulation in reducing postoperative complications

    Objective: To determine based on systematic reviews the effectiveness of early mobilization in reducing complications in postoperative patients. Materials and Methods: A systematic review and meta-analysis examined the Medline, Embase, Cochrane Library, Cochrane Plus, Epistemonikos, and Lilacs databases. Ten articles were selected regarding the effectiveness of early mobilization in reducing complications of all types in postoperative patients. The types of articles were: systematic review 20%, quantitative observational 5%, cross-sectional 10%, randomized clinical trial 5%, longitudinal-descriptive 10%, and longitudinal-comparative 10%; with 10% from the USA, 10% from Chile, 10% from Cuba, 5% from Iran, 5% from Canada, and 5% from Japan. Results: The systematic observational and retrospective review, which synthesizes the results of multiple primary studies, is an essential part of evidence-based nursing due to its rigorous methodology. Of the 7 out of 10 articles reviewed, it is noted that early mobilization helps reduce postoperative complications. The meta-analysis results show a benefit in reducing headaches and lower back pain in patients who receive early mobilization after spinal anesthesia, with high-quality evidence and strong recommendations. Conclusions: Of the 10 articles, 80% (n=7/10) demonstrate the effectiveness of early mobilization in reducing complications in postoperative patients. The research reviewed shows that early mobilization reduces complications of all types in postoperative patients.

    Effectiveness of metformin associated with glibenclamide for the treatment of patients with type II diabetes

    The reviewed investigations highlighted that there is equivalent efficacy and safety in terms of reducing glycosylated hemoglobin between metformin and glibenclamide. Non-inferiority in severe adverse events and cardiovascular events was demonstrated, respectively, in patients receiving glimepiride. The effectiveness varies, and the best cost-effectiveness ratio is found in the metformin regimen.

    Effectiveness of negative pressure therapy to decrease infection of the surgical site of hospital stay for patients in intensive care

    Objective: To analyze the evidence regarding the efficacy of negative pressure therapy to reduce surgical site infection and hospital stay in intensive care patients. Materials and Methods: The research design is a systematic review, with a search conducted in databases: Scielo, Pubmed, Medline, Cochrane Plus, Epistemonikos. The population consisted of 22 articles, and the sample was made up of 10 articles. The articles were evaluated using the GRADE system, which assesses the quality of evidence and strength of recommendation. Results: The scientific evidence comes from countries such as England (20%), followed by the United States (10%), Australia (20%), Sweden (10%), Spain (10%), Italy (10%), Ireland (10%), and Costa Rica (10%). 80% of the articles demonstrate the efficacy of negative pressure therapy in reducing surgical site infection and hospital stay in intensive care patients, while 20% show no evidence of efficacy due to small sample studies. Conclusions: Negative pressure therapy is effective in reducing surgical site infection and hospital stay in intensive care patients.

    Effectiveness of noradrenaline versus dopamine in patients with septic shock that come to the emergency service

    Objective: To analyze and systematize the evidence on the efficacy of dopamine versus norepinephrine for patients with septic shock in the emergency department. Materials and methods: This study is a systematic review. The GRADE system was used to assess the quality of the evidence and grade the strength of the recommendations. The search was conducted in the following databases: LILACS, SciELO, Cochrane Plus, PubMed, MEDLINE, Epistemonikos, and EBSCO. Of the 10 articles systematically reviewed, 30% were systematic reviews and meta-analyses, 20% were clinical trials, and 20% were randomized primary studies. Regarding the country of origin, 30% were from the U.S. and China, and 10% were from France, India, England, and Spain. Results: 90% of the articles agree that norepinephrine is more effective than dopamine, as it has a lower incidence of cardiac-related events compared to dopamine, and 10% found no difference in mortality between the two drugs. Conclusions: It is concluded that norepinephrine is more effective than dopamine in patients with septic shock in the emergency department.

Methodology: An observational and retrospective systematic review, where the search was restricted to full-text articles, and the selected articles underwent critical reading, using the Jover evaluation to identify their level of evidence. Results: In the final selection, 08 articles were chosen. Of these, 40% (3) were from Spain, 30% (2) were from Brazil, and 10% (3) were from other countries, specifically Uruguay, Cuba, and Peru. The majority of the studies were quasi-experimental, accounting for 50%, primarily from Spain. Systematic reviews were conducted in Uruguay and Brazil, accounting for 30%, and 20% were descriptive cross-sectional studies from Spain. Among all the analyzed articles, 80% affirmed that the quality of nursing consultations is effective and efficient in improving health. Conclusions: The reviewed studies provide evidence of the effectiveness of nursing consultation in improving the health of the population, increasing their trust in nursing professionals.

    Effectiveness of nursing interventions to prevent pneumonia in patients with mechanical ventilation.

    Objectives: Systematize studies showing the available evidence on the effectiveness of nursing actions to prevent pneumonia in patients with mechanical ventilation. Materials and Methods: The total population is determined by 15 scientific articles published in documents and disclosed in electronic journals, with an age not exceeding 10 years, and articles published in Spanish and English. The articles observed are of retrospective and descriptive observational design to provide the scientific articles using descriptors such as preventive measures, effectiveness, nursing actions to prevent pneumonia in patients with mechanical ventilation. Results: The articles found demonstrate that they were found in the following databases: Medline, Elsevier, PubMed, Cochrane, Lilacs, Scielo, and were analyzed according to the GRADE scale to determine their strength and quality of evidence. Of the 15 articles reviewed, 60% (9/15) of the articles agree that healthcare staff, nurses, demonstrate effective measures to prevent VAP through scientific knowledge, while 40% (6/15) show that a combination of strategies in nursing activities is essential to prevent VAP, improving care processes to reduce pneumonia. Conclusions: Of the 15 articles reviewed, 80% (12/15) provide evidence that nursing staff adequately comply with the necessary measures due to various experiences and scientific knowledge in preventing pneumonia in patients with mechanical ventilation.

    Effectiveness of quetiapine compared to olanzapine in the decrease of adverse reactions in patients with schizophrenia in the emergency service.

    Objective: Systematize the evidence on the effectiveness of quetiapine compared to olanzapine in reducing adverse reactions in patients with schizophrenia in the emergency department. Materials and Methods: Quantitative study with a systematic review design, the search of 80 was restricted to full-text articles, and the 10 selected articles underwent critical reading, using the GRADE assessment to identify their level of evidence. Results: In the final selection, 10 articles were chosen, with 70% being clinical trials and 30% systematic reviews, 80% of them from Spain, and 20% from Berlin. Conclusions: It was evidenced in 60% of the studies that quetiapine is more effective than olanzapine due to the improvement of cognitive function, reaction quality/attention, shorter interruption time, fewer movement disorders, slight weight gain, fewer parkinsonian effects, and better tolerance; while olanzapine produces weight gain, insulin resistance, metabolic effects, extrapyramidal effects, increased cholesterol and triglycerides, and QT wave alterations.

    Effectiveness of warming devices in the prevention and maintenance of temperature in inadvertent hypothermia in the peri- and postoperative period in surgical patients.

    Objective: To evaluate the evidence on the effectiveness of warming devices in the prevention and maintenance of temperature during inadvertent hypothermia in the perioperative period in surgical patients. Results: Fifty percent (n = 5/10) indicate the effectiveness of warming devices in preventing and maintaining temperature during inadvertent hypothermia in the peri- and postoperative period in surgical patients, while the other 50% (n = 5/10) show concordance but do not provide evidence of effectiveness as it was not measured. Conclusion: Half of the studies (5/10) indicate the effectiveness of warming devices in preventing and maintaining temperature during inadvertent hypothermia in the peri- and postoperative period in surgical patients. The remaining 5/10 indicate concordance with the use of warming devices in the prevention and maintenance of temperature during inadvertent hypothermia in the peri- and postoperative period in surgical patients.

    Efficacy of conventional management compared to unconventional management for the prevention of complications in patients with acute appendicitis

    Efficacy of conventional suture versus staples in post cesarean section patients

    Objective: To analyze and systematize the evidence on the efficacy of conventional sutures versus staples in patients undergoing cesarean surgeries. Materials and Methods: This is a systematic review that included a sample of 10 scientific articles published and indexed in scientific databases such as Cochrane Journal, PubMed, Online Library, and Epistemonikos. Of all the articles included in the study, 7 were conducted in the United States, while 1 was conducted in Nigeria, 1 in China, and 1 in India. Additionally, when grouped by study design, 60% of the articles were meta-analyses, 30% were retrospective cohort studies, and the remaining 10% was a prospective cohort study. Results: 80% of the studies show that conventional sutures are more effective than staples, as they presented fewer complications, less wound dehiscence, better healing in post-cesarean patients, and higher satisfaction, although staples had shorter operative time. 20% of the studies indicate no significant difference between the two groups. Conclusions: It was concluded that conventional sutures are more effective than staples in post-cesarean patients.

    Objective: To analyze and systematize evidence on the effectiveness of enteral administration of lactoferrin for the prevention of sepsis in very low birth weight preterm newborns. Materials and Methods: The study is quantitative, observational, and retrospective with a systematic review design, including a population of 15 articles and a sample of 10 articles. Databases used were articles with full texts, and the selected articles were critically reviewed. The GRADE system was applied to assess the quality of evidence and the strength of the recommendation. Results: 90% of articles demonstrate that enteral administration of lactoferrin in preterm newborns of grade II and III reduces sepsis, and 10% indicate that no favorable results were found to prevent necrotizing enterocolitis in newborns. Conclusions: 9/10 of the studies related to enteral administration of lactoferrin prevent late-onset sepsis and necrotizing enterocolitis in grade II and III preterm newborns with very low birth weight.
