Effectiveness of quetiapine compared to olanzapine in the decrease of adverse reactions in patients with schizophrenia in the emergency service.

Objective: Systematize the evidence on the effectiveness of quetiapine compared to olanzapine in reducing adverse reactions in patients with schizophrenia in the emergency department. Materials and Methods: Quantitative study with a systematic review design, the search of 80 was restricted to full-text articles, and the 10 selected articles underwent critical reading, using the GRADE assessment to identify their level of evidence. Results: In the final selection, 10 articles were chosen, with 70% being clinical trials and 30% systematic reviews, 80% of them from Spain, and 20% from Berlin. Conclusions: It was evidenced in 60% of the studies that quetiapine is more effective than olanzapine due to the improvement of cognitive function, reaction quality/attention, shorter interruption time, fewer movement disorders, slight weight gain, fewer parkinsonian effects, and better tolerance; while olanzapine produces weight gain, insulin resistance, metabolic effects, extrapyramidal effects, increased cholesterol and triglycerides, and QT wave alterations.
Resumen
objetivo: sistematizar las evidencias sobre la efectividad de quetiapina comparado con olanzapina en la disminución de las reacciones adversas en pacientes con esquizofrenia en el servicio de emergencia. materiales y métodos: estudio cuantitativo y diseño de revisión sistemática, la búsqueda de 80 se ha restringido a artículos de texto completos y los 10 artículos seleccionados se sometieron a una lectura crítica, utilizando la evaluación grade para identificar su grado de evidencia. resultados: en la selección definitiva se eligieron 10 artículos, encontramos que el 70% ensayo clínico y 30% revisiones sistemáticas, perteneciendo el 80% corresponden a españa, y el 20% a berlin. conclusiones: se evidenció en el 60% de los estudios, que la quetiapina es más efectivo que la olanzapina debido a la mejora de la función cognitiva de reacción calidad / atención, el tiempo de interrupción es menor, indujo menos trastorno de movimiento, leve ganancia de peso, menos efectos parkinsonianos y mejor tolerado; mientras que la olanzapina produce aumento de peso, resistencia a la insulina, efectos metabólicos, efectos extrapiramidales, aumento de colesterol y triglicéridos, alteración de las ondas qt.
